Le bilan vocal est un moment clé pour la mise en place d'une rééducation orthophonique. En explicitant le lien existant entre la voix et l'état psychologique du sujet, ce mémoire s'interroge sur l'influence que les troubles psychiatriques du patient entretiennent sur la tenue et les résultats du bilan. Des bilans de voix ont été effectués sur quatre patients de l'Unité de Soins Psychiatriques du Centre Hospitalier de Bayonne (64), deux sujets schizophrènes et deux sujets alcooliques. En analysant les résultats des bilans initiaux, réalisés à partir du matériel habituellement utilisé en cabinet libéral, nous nous sommes interrogés sur les informations complémentaires à obtenir pour préciser l'évaluation. De ce questionnement découle un second protocole dont les résultats confirment l 'hypothèse selon laquelle un complément au bilan actuel, prenant en considération la pathologie psychiatrique du patient, ainsi qu'une meilleure collaboration entre psychiatres et orthophonistes, seraient bénéfiques pour le bilan initial et la mise en place d'un plan de rééducation vocale. Enfin, un questionnaire aux orthophonistes en libéral témoigne de la réalité de ces influences et de la demande d'une meilleure collaboration pluridisciplinaire.

Le vieillissement normal entraîne de nombreux changements sur les plans cognitifs, moteurs et linguistiques. Toutefois, l'avancement en âge affecte également une autre facette moins connue de la communication, c'est-à-dire la phonation. Au vu de l'importance de la production de la voix dans la communication verbale humaine, l'objectif de cette thèse était d'examiner l'impact du vieillissement normal et son interaction avec d'autres facteurs sur la production et l'évaluation subjective de la voix. L'exercice du chant et le tabagisme ont été les principaux facteurs étudiés. Quatre études semi-expérimentales transversales de groupe ont été conduites, lors desquelles 267 participants ont produit ou ont évalué des échantillons de voix dans différents contextes. L'Étude 1 visait à examiner l'effet du vieillissement sur la production et l'autoévaluation de la voix. L'Étude 2 avait pour objectif d'étudier l'influence potentiellement positive d'une habitude de vie, l'exercice du chant, sur la relation entre le vieillissement et la voix. L'Étude 3 visait à étudier l'influence potentiellement négative d'une habitude de vie, le tabagisme, sur cette même relation. Finalement, l'Étude 4 avait pour but d'examiner l'effet des facteurs propres au locuteur et à l'évaluateur sur l'évaluation de la voix dans le vieillissement. Les résultats des études confirment que le vieillissement normal a un effet négatif sur la production de la voix tel que mesuré sur différents paramètres acoustiques. Les études démontrent toutefois que l'habileté à moduler l'amplitude et la fréquence de la voix est préservée dans le vieillissement, tant chez les fumeurs que chez les non-fumeurs, du moins jusqu'à 75 ans. De plus, les résultats de l'Étude 2 ont révélé que l'exercice du chant protégeait dans une certaine mesure la voix du déclin de stabilité associé au vieillissement normal. Les résultats de l'Étude 3 ont ensuite confirmé que le tabagisme a de nombreux effets négatifs sur la production de la voix, dont plusieurs sont dépendants de l'âge du locuteur. Les résultats de l'Étude 4 ont montré que l'âge de l'évaluateur a une influence sur l'évaluation de la voix, et que l'influence des facteurs propres au locuteur tels que son âge, son statut tabagique et son sexe est plus grande encore. Les résultats de l'Étude 4 démontrent également que le désir d'interagir avec un locuteur n'est pas associé à l'évaluation de la qualité acoustique de la voix, mais plutôt à son évaluation psychosociale. Ces résultats font échos à l'Étude 1, qui a révélé que l'autoévaluation de la voix est également grandement influencée par des facteurs psychosociaux et peu par la qualité acoustique de la voix. Ces études contribuent à accroître les connaissances sur l'impact du vieillissement normal sur la production de la voix ainsi que sur l'évaluation auditive-perceptuelle et psychosociale de la voix.

What happens – sociologically, linguistically, educationally, politically – when more than one language is in regular use in a community? How do speakers handle these languages simultaneously, and what influence does this language contact have on the languages involved?
