En France, la prévalence de l'obésité des plus de 18 ans est de 17% tous sexes confondus et continue d'augmenter. L'existence de disparités sociales et les liens entre précarité et surcharge pondérale sont actuellement établis. De par la relation singulière qu'il entretient avec ses patients et son rôle dans l'approche bio-psycho-sociale de la santé, le médecin généraliste est placé au cœur de cette prise en charge. L'objectif de notre travail était d'explorer les ressources mobilisées par les médecins généralistes dans la prise en charge des patients obèses en situation de précarité et de déterminer leurs besoins pour soulever des pistes l'amélioration. Treize médecins généralistes exerçant dans la région Auvergne-Rhône-Alpes ont été interrogés lors d'entretiens semi-dirigés. Les médecins généralistes interrogés intègrent les professions paramédicales et les services sociaux et hospitaliers dans leur prise en charge. Ils soulignent le manque de recours extérieurs pour cette pathologie complexe et multifactorielle et évoquent l'importance d'une approche globale et coordonnée entre les différents professionnels de santé. Conscients de l'existence de représentations socio-culturelles liées à l'alimentation, l'adoption d'une stratégie "centrée patient" basée sur l'empathie et la déculpabilisation est l'une des ressources internes importantes mobilisées par les médecins généralistes. Plusieurs structures et plans d'action existent dans la région Auvergne-Rhône-Alpes, l'amélioration de leur diffusion et de leur communication est essentielle et mérite d'être développée. La sensibilisation et la formation des professionnels de santé aux inégalités sociales de santé et à la prévention précoce de la surcharge pondérale grâce à l'éducation thérapeutique et l'entretien motivationnel apparaissent comme des leviers d'amélioration. Le rôle des médecins généralistes semble résider dans l'importance de déstigmatiser sans banaliser.

Determinants of health - like physical activity levels and living conditions - have traditionally been the concern of public health and have not been linked closely to clinical practice. However, if standardized social and behavioral data can be incorporated into patient electronic health records (EHRs), those data can provide crucial information about factors that influence health and the effectiveness of treatment. Such information is useful for diagnosis, treatment choices, policy, health care system design, and innovations to improve health outcomes and reduce health care costs. Capturing Social and Behavioral Domains and Measures in Electronic Health Records: Phase 2 identifies domains and measures that capture the social determinants of health to inform the development of recommendations for the meaningful use of EHRs. This report is the second part of a two-part study. The Phase 1 report identified 17 domains for inclusion in EHRs. This report pinpoints 12 measures related to 11 of the initial domains and considers the implications of incorporating them into all EHRs. This book includes three chapters from the Phase 1 report in addition to the new Phase 2 material. Standardized use of EHRs that include social and behavioral domains could provide better patient care, improve population health, and enable more informative research. The recommendations of Capturing Social and Behavioral Domains and Measures in Electronic Health Records: Phase 2 will provide valuable information on which to base problem identification, clinical diagnoses, patient treatment, outcomes assessment, and population health measurement.

Over the 20th century, Morocco has become one of the world’s major emigration countries. But since 2000, growing immigration and settlement of migrants from sub-Saharan Africa, the Middle East, and Europe confronts Morocco with an entirely new set of social, cultural, political and legal issues. This book explores how continued emigration and increasing immigration is transforming contemporary Moroccan society, with a particular emphasis on the way the Moroccan state is dealing with shifting migratory realities.
