Les vomissements chez le nourrisson sont un motif fréquent de consultation en médecine générale. Nous nous sommes donc intéressé au cas clinique d'un nourrisson de 21 mois présentant des vomissements, chez qui une réhydratation riche en glucose a eu des effets néfastes compte tenu d'un diabète latent. Nous avons donc réalisé d'une part : une étude aux urgences pédiatriques du centre hospitalier de Beauvais pour connaître l'attitude des parents devant les vomissements de leur enfant de moins de deux ans, les raisons, les motifs, les attentes et les craintes vis-à-vis de ce symptôme. D'autre part un questionnaire téléphonique auprès de médecins généralistes centré sur l'utilisation des solutés de réhydratation orale, les conseils diététiques, l'utilisation des thérapeutiques médicamenteuses et l'intérêt d'une fiche d'information destinée aux parents. Après une revue de la littérature, nous tenterons de répondre à une question essentielle : comment peut-on améliorer la prise en charge de l'enfant par ses parents ? Au terme de cette thèse, l'élaboration et la diffusion d'une fiche conseil de conduite à tenir pour les parents devant les vomissements de leur enfant de moins de deux ans, nous parait une des réponses possibles.

Introduction : Près de 63% des recours urgents en ville et à l'hôpital confondus pour des enfants de moins de 2 ans sont motivés par une fièvre. L'évaluation de la pertinence d'un recours aux UP pour fièvre est étudiée mais aucune grille d'évaluation validée n'existe. Nous allons évaluer la pertinence d'une consultation aux UP pour une fièvre de moins de 24h pour un enfant de 3 mois à 6 ans puis nous étudierons les connaissances des parents sur la fièvre. Méthode : Etude prospective observationnelle quantitative aux UP de l'hôpital Yves le Foll à Saint Brieuc. Réalisation d'un questionnaire de recueil d'information et délivrance d'un livret d'information sur la prise en charge de la fièvre. Résultats : Nous avons inclus 78 enfants. 42% des consultations sont jugées pertinentes aux UP par l'examinateur, 27% pour les UP et 27% pour la médecine de ville après relecture des dossiers. 82% des parents donnent du paracetamol en monothérapie. 62% ont découvert leur enfant, 48% l'ont hydraté et 20% aèrent la pièce. 16% appliquent les 3 mesures physiques en même temps. Conclusion : Une échelle d'évaluation de la pertinence des consultations en ville et UP pour fièvre serait à développer. Les connaissances des parents restent moyennes. L'éducation est primordiale.
