Le dispositif des directives anticipées a été introduit dans la législation française le 22 avril 2005, au sein de la loi dite Léonetti. Peu de personnes se saisissent de cet outil. Au-delà du manque d'information, l'objectif de notre étude est de recueillir les représentations des personnes de plus de 75 ans sur la fin de vie, la mort et les directives anticipées et d'évaluer leur impact sur la non-utilisation de ce dispositif. Nous avons réalisé une étude qualitative par entretiens semi-dirigés en face à face auprès de 15 personnes de plus de 75 ans ne présentant pas de maladie rapidement évolutive non contrôlée. La vieillesse favorise le fait de penser à sa propre mort. Les discussions au sujet de la fin de vie et de la mort avec l'entourage sont rares car elles réactivent l'angoisse de la mort chez la personne âgée et génèrent une gêne chez les proches. Pourtant, pour la personne âgée, la famille reste un acteur primordial dans la prise de décisions concernant les soins en fin de vie. Aujourd'hui, le principe d'autonomie de la personne fait peu partie de la culture des personnes âgées. Après avoir été informées elles reconnaissent toutefois un intérêt aux directives anticipées. Le rôle du médecin traitant dans le dispositif des directives anticipées n'est pas perçu comme incontestable. Les conditions actuelles d'exercice, et notamment le manque de temps, entravent l'image de l'implication du médecin traitant. La relation de confiance est un élément important pour favoriser le recueil des directives anticipées par le médecin traitant. Ce travail montre que malgré leurs réticences, pouvoir aborder la question de la mort avec les personnes âgées leur procure de la satisfaction. Le médecin traitant, avec l'appui de l'ensemble des acteurs du parcours de soins, doit permettre de créer le dialogue autour de la fin de vie et de la mort pour favoriser l'utilisation des directives anticipées, qui semblent être un outil intéressant pour la décision éthique, à faire connaître.

Current theories of visual change detection emphasize the importance of conscious attention to detect unexpected changes in the visual environment. However, an increasing body of studies shows that the human brain is capable of detecting even small visual changes, especially if such changes violate non-conscious probabilistic expectations based on repeating experiences. In other words, our brain automatically represents statistical regularities of our visual environmental. Since the discovery of the auditory mismatch negativity (MMN) event-related potential (ERP) component, the majority of research in the field has focused on auditory deviance detection. Such automatic change detection mechanisms operate in the visual modality too, as indicated by the visual mismatch negativity (vMMN) brain potential to rare changes. VMMN is typically elicited by stimuli with infrequent (deviant) features embedded in a stream of frequent (standard) stimuli, outside the focus of attention. In this research topic we aim to present vMMN as a prediction error signal. Predictive coding theories account for phenomena such as mismatch negativity and repetition suppression, and place them in a broader context of a general theory of cortical responses. A wide range of vMMN studies has been presented in this Research Topic. Twelve articles address roughly four general sub-themes including attention, language, face processing, and psychiatric disorders. Additionally, four articles focused on particular subjects such as the oblique effect, object formation, and development and time-frequency analysis of vMMN. Furthermore, a review paper presented vMMN in a hierarchical predictive coding framework. Each paper in this Research Topic is a valuable contribution to the field of automatic visual change detection and deepens our understanding of the short term plasticity underlying predictive processes of visual perceptual learning.
