La décision médicale partagée (DMP) est un concept et un savoir faire, fruit d’une évolution ethno-socio-philosophique s’étendant sur plusieurs décennies. Sa définition n’est pas encore consensuelle et peu d’informations sur sa pratique effective sont aujourd’hui disponibles. De plus, les preuves en sa faveur sont peu étayées dans la littérature scientifique. Cette revue systématique de la littérature étudie les effets de la DMP en termes de qualité, de sécurité, de coût et d’efficacité des soins, en se basant sur une revue Cochrane [50] de 2012. On été incluses, 27 études, antérieure à 2015, répondant à la question posée et dont la définition de la DMP correspondait à celle donnée par les auteurs de la revue. La recherche concernait tous les types d’études et les revues systématiques. Les résultats et la qualité des études ont été comparés. Des résultats significatifs en terme d’amélioration de satisfaction (grade B), d’adhérence au traitement (grade B) et de qualité de vie du patient (grade B) ont été retenus. On note également une plus grande efficacité et moins d’abandon de traitement (grade A). Les études sélectionnées étaient en général de bonne qualité, mais de puissance faible. Les contextes de soin et les pathologies rencontrées étaient variés ; La plupart étaient en faveur de la pratique de la DMP, surtout en médecine ambulatoire et en psychiatrie. Cependant, l’absence de consensus sur la définition de la DMP rend difficile la comparaison entre les études et son mode d’évaluation privilégié reste encore à trouver. Une réflexion sur la formation apportée aux professionnels de santé en matière de DMP semble nécessaire. L’adaptabilité est à privilégier.

An estimated 69000 people die each year from opioid overdose. Opioid overdose is easily reversed with the opioid antidote naloxone and with basic life support. Such care is generally only available in medical settings, however. These guidelines recommend that people who are likely to witness an opioid overdose, including people who use opioids, and their family and friends should be given access to naloxone and training in its use so that they can respond to opioid overdose in an emergency if a medical response is not available. Naloxone can be injected or administered intra-nasally and has minimal effects in people who have not used opioids. While naloxone administered by bystanders is a potentially life-saving emergency interim response to opioid overdose, it should not be seen as a replacement for comprehensive medical care.
