L'échographie est un outil d'imagerie médicale utilisée par certains médecins, en complément de l'examen clinique, afin d'améliorer le diagnostic et la prise en charge du patient. Il n'existe quasiment pas de formation échographique spécifique adaptée à la pratique de cette technique en médecine générale. Nous avons cherché à évaluer les besoins des généralistes en échographie afin d'envisager une formation ciblée sur les échographies les plus utiles en soins primaires. Une étude descriptive dont l'objectif était de décrire les examens échographiques prescrits en médecine générale a été réalisée. Le recueil de données prospectif, réalisé auprès de 32 médecins généralistes des régions PACA et Corse sur 3 semaines, concernait leurs prescriptions échographiques et les pathologies recherchées. Une étude qualitative, dont l'objectif était d'identifier auprès de généralistes, les échographies utiles de savoir pratiquer eux mêmes, a été réalisée suivant la méthode du groupe nominal. Sur 8122 consultations, le taux de prescription d'échographies était de 5,1%, IC 95% (4,6% - 5,6%).Les examens les plus prescrits étaient les échographies de l'appareil locomoteur (23%), les échographies abdominales (14%) et réno-vésicales (12%). sur les 76 pathologies recherchées par ces examens, les plus fréquentes étaient : les tendinopathies de l'épaule, les thromboses veineuses profondes et les lithiases biliaires. Pour 63% des médecins interrogés l'échographie avait sa place en cabinet de médecine générale et 44% d'entre eux étaient intéressés par une formation adaptée à la médecine générale. Les généralistes des 2 groupes nominaux ont identifié et hiérarchisé 18 pathologies correspondant aux échographies jugées utiles de pratiquer eux -mêmes. L'échographie est un outil qui intéresse les généralistes et pour lequel une formation spécifique pourrait être envisagée. Nos résultats proposent des pistes de réflexions quand aux examen les plus utiles en médecine générale

L'échographie est un examen idéal de premier recours, il est rapide, non invasif et de plus en plus performant. Afin de proposer la formation la plus adaptée possible à la médecine générale, il convient d'observer la pratique des différents généralistes utilisant déjà l'échographe. L'objectif principal de cette étude était de décrire quelles étaient les principales indications de l'échographie et/ou de l'échoscopie en médecine générale et d'essayer de savoir en quoi cet examen pouvait modifier la prise en charge des patients. C'était une enquête observationnelle épidémiologique, descriptive, transversale, multicentrique. Cinq médecins généralistes échographistes remplissaient un questionnaire après la réalisation de chaque examen. 2 ont finalement été inclus. 184 questionnaires ont été recueillis sur 3 mois, 150 ont été analysés. Les types d'examens les plus fréquemment réalisés ont été abdominal (42,6 %), vasculaire (16 %) et gynécologique (16%) et les principales indications étaient en accord avec la liste proposée dans « Sonosthéto ». 67,8% des examens ont permis une modification de la prise en charge. Le type de modification le plus fréquemment retrouvé est l'instauration ou la modification d'un traitement. L'échographie semble avoir toute sa place en médecine générale. Il est donc nécessaire de développer cette compétence, et d'aider les médecins généralistes à l'acquérir en leur proposant une formation adaptée à leur pratique.
