L’utilisation de sources laser femtoseconde permet d’améliorer les performances analytiques du couplage, l’interaction laser-matière privilégiant les phénomènes athermiques contrairement aux lasers nanoseconde plus classiquement utilisés. Une collaboration entre le Laboratoire de Chimie Analytique Bio-Inorganique et Environnement de Pau et Novalase SA (spécialisé dans l’intégration optique et laser) a fait naitre la première station d’ablation laser femtoseconde intégrée. Ses caractéristiques particulières, que sont la haute cadence de tir et un optique de balayage rapide du faisceau, permettent d’explorer de nouvelles stratégies d’ablation et ouvrent des perspectives analytiques encourageantes en terme de sensibilité et de mode de quantification. Le potentiel de ce prototype a été évalué au travers d’analyses fondamentales, telles que le fractionnement élémentaire et l’étude de la taille des particules de l’aérosol produit par ablation, et de différentes applications telles que l’analyse de sols et de sédiments par dilution isotopique réalisée par le laser dans la chambre d’ablation, la détection de sélénoprotéines dans des gels d’électrophorèse et la détermination de l’isotopie de l’uranium dans des particules micrométriques.

INTRODUCTION : la mucite orale (MO) est une complication fréquente chez les patients atteints d'un cancer ORL, traités par radiochimiothérapie. Des soins de support telle la laserthérapie de basse énergie (LLLT) pourraient réduire l'incidence et la sévérité des MO. L'efficacité de la LLLT a été évaluée dans cet essaicontrôlé randomisé en triple aveugle. PATIENTS ET METHODES : cet essai de phase III concernait des patients atteints d'un cancer ORL (cavité orale, oro/hypopharynx, stade III ou IV), recevant une radiothérapie conformationnelle (3D ou modulation d'intensité) et une chimiothérapie concomitante (cisplatine ou carboplatine, 5-fluorouracile, ou cetuximab). Une randomisation équilibrée était effectuée : les sujets bénéficiaient d'une laserthérapie (active 4 J/cm2 ou placebo) sur les lésions de MO grade e"2, jusqu'à résolution. L'évaluation clinique hebdomadaire de la cavité orale était effectuée en aveugle. Le critère de jugement principal était la sévérité de la MO (incidence et durée des grades e"3). RESULTATS : parmi les 97 patients inclus et randomisés, seuls 83 (85,6%) ont été évalués en fin de traitement (erreurs d'inclusions et interruptions précoces de la radiochimiothérapie). L'équilibre de randomisation était maintenu et les caractéristiques de la population (sex-ratio, moyenne d'âge, schémas de radiochimiothérapie, incidence des toxicités) étaient comparables dans les deux bras. Une MO sévère (grade e"3) était observée chez 41 patients (49,4%) : 23 (54,8%) du bras actif contre 18 (43,9%) du bras contrôle (intention de traiter modifiée, p=0,32). Le délai médian de survenue d'une MO sévère chez la moitié des patients était de 8 semaines (bras actif) versus 9 semaines. Pour la mucite grade e"2, 68 patients (81,9%) étaient concernés, avec un délai d'apparition de 5 semaines (bras actif) versus 6. Parmi 83 patients, 32 n'ont eu aucune LLLT (active ou placebo), à cause de lésions de MO inaccessibles. L'analyse per protocole montrait que 70,6% des patients avaient un grade e"3 de MO (p=0,75). Le laser était très bien toléré chez 95% des patients. CONCLUSION : cette étude a évalué la laserthérapie recommandée (4 J/cm2), mais manque de puissance. Malgré des données de tolérance encourageantes, de larges études de supériorité devront être menées
