La technique du ganglion sentinelle est devenue récemment une référence dans les cancers du sein et les mélanomes. Son rôle possible dans les cancers gynécologiques (cancer du col de l'utérus, de l'endomètre et de la vulve) est en cours d'investigation. La technique est d'ores et déjà appliquée dans certains cancers du col de l'utérus et de la vulve par les équipes qui en ont l'expérience. Nous avons souhaité analyser les facteurs qui peuvent influencer sa réussite par une étude rétrospective sur les examens réalisés aux Hospices Civils de Lyon de 2005 à 2014. Nous avons identifié des facteurs potentiellement impliqués dans la réussite de l'examen et les avons extraits des dossiers des patientes chez qui un ganglion sentinelle pelvien avait été recherché. L'influence de ces facteurs a ainsi pu être confrontée aux résultats de détection préopératoire et peropératoire de recherche du ganglion sentinelle. Les taux de détection retrouvés ont été respectivement de 93,6 %, 87,8 % et 84,6 % respectivement dans les cancers du col de l'utérus, de l'endomètre et de la vulve. Dans les cancers du col de l'utérus, une différence significative de détection a été observée lorsque le médicament radiopharmaceutique (Nanocis® ou Nanocoll®) variait et quand le protocole long était appliqué plutôt que le protocole court. Dans les trois cancers, nous avons pu observer des taux de détection préopératoires s'améliorant lorsque la détection couplait TEMP et TDM plutôt que lors de la réalisation d'une scintigraphie planaire seule. Nous avons également pu observer une amélioration de la détection peropératoire combinée, colorimétrique et visuelle, par rapport à ces deux modalités réalisées seules dans chacun des cancers. Ces résultats ne sont cependant pas significatifs. Enfin, aucune modification de la détection ne semble être retrouvée lors de la modification de l'activité ou du volume de médicament radiopharmaceutique administré, ni lors de la réalisation d'une conisation dans les cancers du col de l'utérus. Les taux de détection retrouvés ont été conformes aux données actuelles de la littérature pour les cancers du col de l'utérus et de l'endomètre; ils ont été inférieurs dans les cancers de la vulve. Les différences de taux de détection retrouvées dans les protocoles longs et lors des modifications de médicament radiopharmaceutique semblent pouvoir être expliquées par la différence de taille des colloïdes des deux médicaments radiopharmaceutiques qui modifie leur cinétique. La variation des taux de détection selon les modalités préopératoire et peropératoire de recherche du ganglion sentinelle est conforme aux données de la littérature. L'évaluation de cette technique doit se poursuivre afin d'harmoniser les pratiques et ainsi établir des recommandations

However, the recognition that prognostic factors often have a greater impact on outcome than available therapies and the proliferation of biochemical, molecular, and genetic markers have resulted in renewed interest in this field. The outcome in patients with cancer is determined by a combination of numerous factors. Presently, the most widely recognized are the extent of disease, histologic type of tumor, and treatment. It has been known for some time that additional factors also influence outcome. These include histologic grade, lymphatic or vascular invasion, mitotic index, performance status, symptoms, and most recently genetic and biochemical markers. It is the aim of this volume to compile those prognostic factors that have emerged as important determinants of outcome for tumors at various sites. This compilation represents the first phase of a more extensive process to integrate all prognostic factors in cancer to further enhance the prediction of outcome following treatment. Certain issues surround ing the assessment and reporting of prognostic factors are also considered.
