In May 2014, the French research laboratory ISEOR (Socio?Economic Institute for Firms and Organizations) and the University of St. Thomas co?sponsored a second conference on the application of the Socio?Economic Approach to Management (SEAM) paradigm and methodology in the United States. SEAM is a scientific approach to consultancy that focuses on uncovering the dysfunctions and hidden costs that exist in organizations, “hidden” in the sense that they are not captured by traditional accounting methods and financial analyses. Through intervention that encompasses the entire organization – what the ISEOR team refers to as the HORIVERT approach (combining horizontal and vertical intervention) – the underlying goal is to enhance organizational performance by attacking the “TFW virus,” a vestige of the early work by Frederick Taylor, Henri Fayol, and Max Weber that has sufficiently infiltrated our thinking about management and organization to the point where are falling well short of our own potential. The rationale behind all these congresses is the scientific need to share results and ideas and the educational need to train a wide variety of professional staff in the potential of health care and medical informatics. All the caring professions are involved, doctors, scientists, nurses, para-medical staff, administrators, health care planners, community physicians, epidemiologists, statisticians, operations analysts together with specialists from the computing profession dealing with system analysis, hardware, software, languages, data-bases and the marketing of systems. Medical Informatics is a very wide subject with ramifications throughout the health care and preventive services; it offers a key to the monitoring and improvement of patient care and to the provision of a healthier environment. The collection and evaluation of relevant data improves our understanding of the ways in which health care is provided while the availability of cheaper computer hardware and more versatile software enables us to design and implement more revealing and intelligent medical systems. Even though typical systems take a substantial amount of time to design, implement and evaluate, there is the continuing need for informaticians to assess the current state of developmen.

Ainsi, la couverture effective — une mesure de la prestation de services qui répond à une norme minimale de qualité — demeure à un niveau inacceptable. Le rapport Améliorer la couverture effective en matière de santé examine une approche particulière visant à améliorer la couverture effective : les incitations financières sous la forme d’un financement basé sur la performance (FBP), une réforme globale qui comprend généralement la rémunération liée à la performance pour les agents de santé de première ligne ainsi que l’autonomie des établissements, la transparence et l’engagement communautaire. Ce rapport de recherche s’appuie sur un vaste ensemble d’études rigoureuses et de nouvelles analyses. Par rapport au statu quo, dans les pays à faible revenu dotés de systèmes de santé centralisés, le FBP peut entraîner des gains substantiels en matière de couverture effective. Toutefois, les avantages relatifs du FBP — la composante rémunération liée à la performance en particulier — sont moins clairs lorsqu’il est comparé à deux autres approches, à savoir : le financement direct des établissements, qui permet d’octroyer des budgets de fonctionnement aux services de santé de première ligne en accordant aux établissements l’autonomie en matière d’allocations budgétaires, mais sans rémunération liée à la performance ; et le soutien financier lié à la demande pour les services de santé (c’est-à -dire les transferts monétaires conditionnels et les vouchers). Bien que le FBP se traduise souvent par des améliorations marginales, combler les lacunes importantes de la couverture sanitaire effective n’est pas encore à la portée de nombreux pays. Néanmoins, d’importantes leçons et expériences tirées de la mise en oeuvre du FBP au cours de la dernière décennie peuvent orienter le financement de la santé à l’avenir. En particulier, pour réussir, la réforme du financement de la santé devra sans doute moins se focaliser sur la rémunération à la performance tout en conservant les éléments de financement direct des établissements, l’autonomie, la transparence et l’engagement communautaire.

Many patients who present to district (first-referral) level hospitals require surgical treatment for trauma, obstetric, abdominal or orthopedic emergencies. Often surgery cannot be safely postponed to allow their transfer to a secondary or tertiary-level hospital but many district hospitals in developing countries have no specialist surgical teams and are staffed by medical, nursing, and paramedical personnel who perform a wide range of surgical procedures often with inadequate training. The quality of surgical and acute care is often further constrained by poor facilities, inadequate low-technology apparatus and limited supplies of drugs, materials, and other essentials. The mission of the team responsible for Clinical Procedures in the World Health Organization Department of Essential Health Technologies (EHT) is to promote the quality of clinical care through the identification, promotion and standardization of appropriate procedures, equipment and materials, particularly at district hospital level. WHO/BCT has identified education and training as a particular priority, especially for non-specialist practitioners who practice surgery and anesthesia. It has therefore developed Surgical Care at the District Hospital as a practical resource for individual practitioners and for use in undergraduate and postgraduate programs in-service training and continuing medical education programs. The manual is a successor of three earlier publications that are widely used throughout the world and that remain important reference texts: General Surgery at the District Hospital (WHO 1988), Surgery at the District Hospital: Obstetrics Gynecology Orthopedics and Traumatology (WHO 1991), Anesthesia at the District Hospital (WHO 1988; second edition 2000). This new manual draws together material from these three publications into a single volume which includes new and updated material, as well as material from Managing Complications in Pregnancy and Childbirth: A Guide for Midwives and Doctors (WHO 2000).
