Afin de concrétiser l'engagement gouvernementale d'un accès à des soins d'urgence en moins de 30 minutes pour tout citoyen sur le territoire français, le système des médecins correspondants du SAMU (MCS) a été promu. Ces médecins généralistes (MG), formés à l'urgence, constituent un moyen de premier recours compétent et efficient dans les zones où ils sont déjà en place. En Auvergne, 7% de la population est hors délai en cas d'urgence vitale. Il n'existe pourtant qu'un seul réseau départemental de correspondants actifs. Nous avons cherché à comprendre pourquoi les MG des zones concernées renoncent à s'engager dans ce dispositif. Etude qualitative par entretiens semi directifs auprès d'un échantillon raisonné de 16 médecins généralistes des 4 départements de la région Auvergne exerçant à plus de 30 minutes d'une structure de soins d'urgence. Deux types de freins coexistent. Les uns, indépendants des médecins, sont en rapport avec un défaut de communication sur le sujet, un manque de formation initiale, et une nécessaire revalorisation du statut de médecin"formé à l'urgence". Les MG se questionnent sur la non-utilisation des moyens existants. Les autres émanent des médecins eux-mêmes et de leurs représentations du métier de généraliste. Les contraintes de répondre à une demande de soins croissantes en libéral, le sentiment de dépossession de cette activité d'urgence et la rupture avec la médecine hospitalière freinent leur engagement. Au delà des contraintes d'organisation, de rémunération et d'efficacité du médecin correspondant du SAMU, les médecins généralistes ne s'approprient plus cette activité d'urgence qu'ils laissent à la pratique hospitalière. Les besoins qu'imposent l'activité libérale ne leur permettent plus d'accumuler des fonctions supplémentaires. La campagne de déploiement des médecins correspondants du SAMU devra donc s'attacher à réconcilier ces médecins avec l'urgence et leur assurer les meilleures conditions possibles d'exercice.

Liberty Fund's new six-volume The Collected Works of Frederic Bastiat series, of which "The Man and the Statesman "is the first volume, may be considered the most complete edition of Bastiat's works published to date, in any country, and in any language. The main source for this translation is the seven-volume "Oeuvres completes de Frederic Bastiat," published in the 1850s and 1860s. The present volume, most of which has never before been translated into English, includes Bastiat's complete correspondence: 207 letters Bastiat wrote between 1819, when he was only 18 years old, until just a few days before his untimely death in 1850 at the age of 49. For contemporary classical liberals, Bastiat's correspondence will provide a unique window into a long-forgotten world where opposition to war and colonialism went hand-in-hand with support for free trade and deregulation.
