Un relais dans la prise en charge pédiatrique est nécessaire pour les adolescents ou jeunes adultes atteints de pathologie chronique. La transition est définie par la progression du milieu de soin pédiatrique vers les équipes de soins adultes. Ce processus est complexe car il intervient chez des individus en pleine mutation qui ont des besoins spécifiques relatifs à l'adolescence. Les principaux objectifs de la transition sont d'assurer une continuité des soins et de limiter les ruptures de suivi fréquentes à cette période. L'objectif de cette étude était de décrire le déroulement de la transition chez 48 patients diabétiques de type 1 par l'analyse de leurs paramètres médicaux deux années avant et deux années après ce relais, et par une enquête qualitative. A 2 ans de la transition, 8% des patients n'avaient pas de suivi médical et nécessitaient des hospitalisations régulières pour décompensation du diabète et 15% étaient suivis de façon aléatoire par leur médecin traitant. Les déséquilibres du diabète (HbA1c> 9%) concernaient 25% de la population. Plus de la moitié des patients déséquilibrés sur le plan du diabète l'étaient déjà en fin de prise en charge pédiatrique. Les hospitalisations pour acidocétose étaient multipliées par deux mais concernaient un nombre limité de patients (10 %) qui étaient hospitalisés de façon récurrente (2,4 hospitalisations par année). Les patients avaient moins de consultations médicales (3,6 consultations par année un an avant le passage contre 1,6 par année deux ans après) et étaient moins suivis sur le plan psychologique. Les patients ont proposés dans l'enquête que des structures de soins spécifiques pour adolescents soient créées. L'intérêt serait d'aider les jeunes patients à adhérer au suivi médical et de les préparer à une prise en charge future en milieu de soins adultes. Des études à plus grande échelle, prospectives, seront nécessaires afin d'évaluer l'efficacité de programmes de transition et notamment de structures médicales pour adolescents.

The U.S. population is aging, and occupational therapy practitioners working with older adults must have the skills to address multiple health issues, including vision loss. Although typical aging does not cause low vision, four major causes of vision impairment are directly related to the aging process: (1) age-related macular degeneration (AMD), (2) diabetic retinopathy, (3) glaucoma, and (4) cataracts. Instances of these impairments are expected to double by the year 2050, making low vision a significant public health issue. This Practice Guideline includes occupational therapy assessment and intervention guidelines for older adults who have visual acuity impairments, visual field impairments, or both as a result of a low vision diagnosis. Screening, referral, and evaluation are covered for clients for central visual field impairments, such as AMD and cataract; peripheral field impairments, such as glaucoma, hemianopsia, and quadrantanopsia; and mixed visual field loss, such as diabetic retinopathy. This work can help occupational therapists and occupational therapy assistants, as well as individuals who manage, reimburse, or set policy regarding occupational therapy services, understand the contribution of occupational therapy to treating older adults with low vision. Appendixes include selected diagnostic and billing codes and evidence tables.

Central banks came out of the Great Recession with increased power and responsibilities. Indeed, central banks are often now seen as 'the only game in town', and a place to put innumerable problems vastly exceeding their traditional remit. These new powers do not fit well, however, with the independence of central banks, remote from the democratic control of government. Central Banking in Turbulent Times examines fundamental questions about the central banking system, asking whether the model of an independent central bank devoted to price stability is the final resting point of a complex development that started centuries ago. It dissects the hypothesis that the Great Recession has prompted a reassessment of that model; a renewed emphasis on financial stability has emerged, possibly vying for first rank in the hierarchy of objectives of central banks. This raises the risk of dilemmas, since the Great Recession brought into question implicit assumptions that the pursuit of price stability would also lead to financial stability. In addition, the border between monetary and fiscal policy was blurred both in the US and in Europe. Central Banking in Turbulent Times asks whether the model prevailing before the Great Recession has been irrevocably altered. Are we entering, as Charles Goodhart has hypothesized, into the 'fourth epoch' of central banking? Are changes to central banks part of a move away from the global liberal order that seemed to have prevailed at the turn of the century?
