La transplantation rénale apporte de meilleurs résultats en termes de survie et de qualité de vie que les techniques d'hémodialyse. Néanmoins, le traitement de la greffe et les différentes complications pouvant survenir montrent que le patient transplanté nécessite une attention particulière des professionnels de santé. La revue de la littérature réalisée montre que le rôle du pharmacien est primordial auprès des patients transplantés rénaux. L'importance de son implication s'explique par la complexité du traitement de la greffe, ses effets indésirables, le risque d'interactions médicamenteuses et par la nécessité d'un bon usage du médicament pour la survie du greffon. La prise en charge de ces patients est technique et souvent réservée aux services spécialisés. Le pharmacien d'officine a peu de contact avec les autres professionnels de santé, alors que sa disponibilité et ses connaissances en font souvent un des premiers recours du patient. Dans le cadre de la loi HPST, la coopération entre professionnels de santé, et le lien ville-hôpital doivent être développés pour favoriser la prise en charge globale du patient. L'objectif de cette thèse d'exercice est d'établir une coopération, pour la prise en charge du patient transplanté rénal, entre le service de Néphrologie du CHU de Nantes et le réseau officinal, en développant un outil informatique de communication et de partage d'informations. Cette coopération est subordonnée à la sensibilisation et à l'information des pharmaciens d'officine.

Pour cela l'équipe officinale doit tout d'abord être formée à cette stratégie thérapeutique immunosuppressive. C'est pourquoi cette thèse présente successivement les différents immunosuppresseurs disponibles en ville. Ces derniers sont résumés dans une « fiche mémo » à afficher ou garder dans son officine. Ensuite, les pharmaciens d'officine pour accompagner leurs patients greffés du rein peuvent effectuer, selon leur volonté, des entretiens pharmaceutiques dont la mise en place et le déroulé sont également proposés.
