On estime que dans les stades évolués de ces pathologies, près d'un patient sur deux souffre de douleurs chroniques. Le diagnostic et le traitement de la douleur dans cette population fragile représente donc un enjeu majeur de santé publique.Les troubles neurocognitifs sévères altèrent les capacités de communication. Chez les patients atteints de ces troubles, la douleur est donc souvent sous-exprimée, sous-diagnostiquée et insuffisamment traitée. De plus, elle s'accompagne fréquemment de troubles psycho-comportementaux, comme l'agitation ou l'anxiété, qui complexifient le repérage de la douleur et peuvent être à l'origine de prescriptions inappropriées de psychotropes.Pour évaluer la douleur chez ces patients, nous avons vu que l'échelle MOBID-2 a montré son efficacité. Les échelles NPI et CMAI permettent quant à elles une évaluation spécifique des troubles du comportement. Sur le plan thérapeutique, il est recommandé de débuter l'antalgie par du Paracétamol, puis par des Morphiniques, sans recourir aux antalgiques de palier 2. En cas de douleurs neuropathiques associées, l'utilisation de la Prégabaline est recommandée.Cette prise en charge permet une antalgie efficace; une réduction des troubles psychocomportementaux et une amélioration de la qualité de vie des patients.
