Chaque année en France, environ 2600 nouveaux cas de cancer sont diagnostiqués chez les enfants et les adolescents. L'annonce du diagnostic de cancer est un évènement traumatique et angoissant, provoquant un bouleversement médico-psycho-social majeur, concernant à la fois l'enfant ou l'adolescent lui-même mais également ses parents. C'est dans ce contexte que la mesure 40 du 1er plan cancer garantissant l'accès à un dispositif d'annonce ainsi que les recommandations d'organisation des soins en cancérologie pédiatrique, ont vu le jour en 2003 et 2004. L'objectif de notre étude était d'évaluer l'adéquation entre le contenu du dispositif d'annonce mis en œuvre dans le service d'hématologie et oncologie pédiatrique du CHU d'Amiens avec les recommandations fixées par l'Institut National du Cancer (INCa), à travers des témoignages d'enfants et adolescents suivis dans le service mais également de leurs parents et des médecins, et de recueillir leur ressenti. Étude monocentrique rétrospective, avec questionnaires élaborés autour des items développés par l'INCa dans son plan d'annonce du diagnostic de cancer et remis aux enfants, adolescents et parents suivis au CHU d'Amiens de janvier 2013 à juin 2018. Parmi les 109 questionnaires, 72% nous avaient été retournés. Les enfants, adolescents et parents avaient eu accès à une consultation d'annonce dans 95% des cas, réalisée dans la totalité des cas par un médecin spécialisé en cancérologie pédiatrique. Globalement, ils se déclaraient tous satisfaits des modalités d'annonce et du contenu des informations. Cependant, un grand nombre d'entre eux estimaient ne pas avoir posé l'ensemble de leurs questions, principalement car elles étaient apparues secondairement, probablement en rapport avec le mécanisme de sidération induit par le choc de l'annonce d'où l'importance du temps d'accompagnement soignant prévu par l'INCa. Les soins de supports avaient été proposés à 98% des parents mais leur recours était variable, essentiellement représenté par la prise en charge psycho-sociale. Concernant le lien avec la médecine de ville, les parents comme les praticiens du service souhaitaient impliquer d'avantage les médecins généralistes dans la prise en charge de ces enfants malades. Le dispositif d'annonce du cancer a pour objectif d'atténuer l'effet traumatique et les répercutions médico-psycho-sociales de cette annonce. La particularité de la pédiatrie, prenant en charge les enfants et adolescents de 0 à 18 ans, doit adapter les informations à leur âge et leur stade de développement, en tenant compte de la place des parents. Cependant, au-delà de l'annonce de la maladie, les explications fournies de manière continues aux enfants, aux adolescents et à leurs parents aux différentes étapes de la maladie contribuent à l'alliance thérapeutique et renforcent leurs capacités de résilience.

Introduction : L’annonce diagnostique est un moment particulièrement difficile à vivre pour les enfants atteints de cancer et leurs parents. Depuis plusieurs années, de nombreuses recommandations ont été publiées afin de guider la pratique de telles annonces. Objectif : L’objectif de cette étude était d’évaluer la qualité et le vécu parental de l’annonce diagnostique dans le service d’hémato-oncologie pédiatrique du CHU de Reims entre 2011 et 2016. Résultats : 138 parents de 92 enfants suivis dans le service ont été inclus. Les parents interrogés étaient globalement très satisfaits de la façon dont s’est déroulée cette annonce et des informations qui leur ont été transmises. Le mot cancer n’était prononcé que dans 52% des cas. Les sujets abordés tournaient essentiellement autour de la maladie et de son traitement médical. Les prises en charges sociales, psychologiques et scolaires étaient moins souvent abordées. La disponibilité et le soutien reçu de la part des médecins et infirmières était jugée très satisfaisants. Les difficultés sociales (baisse des revenus et difficultés financières, isolement social...) étaient fréquentes (47%) et leur prise en charge était jugée globalement satisfaisante. Les difficultés psychologiques (troubles anxieux, dépression, troubles du sommeil...) étaient également fréquentes (68%). Le choc émotionnel provoqué non seulement par l’annonce était important avec des scores d’affects négatifs élevés après l’annonce. La prise en charge psychologique était souvent jugée insuffisante par les parents. Conclusion : Même si les parents sont satisfaits de la façon dont elle s’était déroulée, l’impact de l’annonce diagnostique est important en oncologie pédiatrique. Certaines améliorations telles que l’augmentation du temps alloué à la psychologue dans le service, la meilleure formation des médecins, l’intervention d’une infirmière d’annonce ou la remise de documents d’information plus complets pourraient permettre de réduire l’impact de l’annonce sur le fonctionnement des parents d’enfants atteints de cancer.
