Introduction : Plus de 120 000 prothèses de hanche sont posées chaque année en France et la chirurgie de reprise représente actuellement environ 15% des prothèses implantées. Bien que la technique de mise en place d'une prothèse de première intention soit aujourd'hui bien standardisée, il n'en est pas de même pour les reprises. Les avis s'opposent avec des concepts différents et des résultats peu satisfaisants. Matériel et méthodes : La série rétrospective comprenait 158 reprises de prothèses totales de hanche chez 153 patients (130 bipolaires, 20 uniquement fémorales et 8 uniquement acétabulaires) opérés entre janvier 2004 et mars 2010. L'âge moyen était de 73 ans. Parmi les 138 changements d'implant acétabulaire, 55 ont bénéficié d'un implant simple mobilité, 72 d'un implant double mobilité et 11 d'un implant simple mobilité associé à une collerette. Résultats : Le recul moyen était de 2,46 ± 1,77 années (31 mois ± 21 mois). 17 patients sont décédés, 20 perdus de vue. Nous avons observé 12 implants qui sont luxés au moins une fois. La proportion de luxation post opératoire est donc de 7,6%. Sur des 12 prothèses, il s'agissait 11 fois d'une cupule standard et une fois d'une cupule double mobilité. Nous avons dénombré 10 cas de révisions majeures au dernier recul, le taux de survie actuarielle des nouveaux implants à 6,5 ans en prenant comme définition de l'échec la reprise chirurgicale avec changement de l'un des deux implants est de 89% (IC 95% : 85,4-93,3). Discussion : Concernant le sous-groupe des reprises pour luxations itératives, le taux d'instabilité et de descellement aseptique était nul lors de l'utilisation de l'implant double mobilité. Concernant le sous-groupe des reprises pour descellement aseptique, le taux de l'instabilité et de descellement aseptique était de 1,8% lors de l'utilisation de l'implant double mobilité. Toutes indications de RPTH confondues, la survie (en prenant comme définition de l'échec la reprise chirurgicale pour descellement de la cupule) de l'implant double mobilité est de 90,28% (IC 95% : 94,3-98,9%) à 5,3 ans. Conclusion : Les résultats de l'implant de double mobilité, en terme de fixation et de stabilité, comparés aux implants simple mobilité et aux implants contraints confirment son indication dans les reprises d'arthroplastie totale de hanche pour descellement et instabilité.

Les reprises des prothèses totales de hanche de première intention sont un enjeu d'avenir pour l'orthopédie, le nombre d'cc étant en constante augmentation en France, posées chez des patients de plus en plus jeune et à l'espérance de vie toujours plus longue. Notre participation à une étude prospective multicentrique, qui sera présentée lors du prochain congrès annuel de la Société Française de Chirurgie Orthopédique et Traumatologique,nous a permis de suivre au CHU de Nantes 90 patients ayant nécessité un changement de leur prothèse de hanche.Les principales causes d'échec retrouvées étaient les descellements aseptiques (24%), les fractures périprothétiques (16%), les luxations (16%), les infections (13%), les ostéolyses et usure (11%), les fractures d'implant (11%) et les erreurs techniques (4%).Nous avons alors recherché les nombreux facteurs, qu'ils soient propres au patient ou à sa prothèse, qui ont pu influencer la survenue de ces échecs. Les modalités de la reprise ont également été étudiées. A l'issu de ce travail, nous proposons une réflexion dans les choix qu'un chirurgien doit faire, tant lors d'une arthroplastie totale de hanche de première intention que dans une chirurgie de reprise.

The theme of this work is the application of the engmeermg theory of frictional torque to total hip replacement. The author adhered tenaciously to this theory, involving the use of a small-diameter femoral head, throughout the epoch when the large-diame ter, metal-to-metal design dominated the field. During that considerable period general satisfaction with the early results rendered criticisms of the large-diameter head unwelcome. There was a formidable array of counter criticism: the small head would pierce a film of synovial fluid; the small head would wear the socket too rapidly; the small head would always have a high risk of dislocation; detachment of the trochanter, to achieve precise orientation for the small head, was unacceptable. But all these objections have now been largely overcome. Lubrication of high molecular weight polyethylene (HMWP) on metal is now accepted as being mainly by the boundary regime with thick fluid films playing no part. We now know that HMWP can indeed tolerate the very high stresses imposed by the small head and in tribological theory there may even be some advantage in high stress. Dislocation is now known not to be an automatic sequel to the small head.
