Dès les premières années, un enfant construit les préférences alimentaires et habitudes de vie qui auront un impact sur sa corpulence. Objectifs : Evaluer la prévalence du surpoids chez les enfants suivis en consultation de Protection maternelle et infantile (PMI) au moment de la cassure physiologique de la courbe de corpulence. Observer l’évolution de cette prévalence entre 2000 et 2010. Méthode : Etude rétrospective transversale répétée incluant 172 enfants nés en 1997, 2002 et 2007 suivis entre l’âge de 13 et 30 mois dans un centre de consultation de Dreux. Résultats : Nous avons observé une diminution significative de la prévalence du surpoids entre 2000 et 2010. 30,8% (IC95% [19,1–45,3]) des enfants nés en 1997 ont été en surpoids entre l’âge de 13 et 30 mois contre 13% (IC95% [5,4–27,0]) des natifs de 2007. A l’âge de 30 mois, 23% (IC95% [13,0–37,2]) des enfants nés en 1997 étaient en surpoids tandis que tous les enfants nés en 2007, inclus dans l’étude, avaient corrigé leur surpoids au même âge (IC95% [0-2,6]). Conclusion : La diminution de la prévalence du surpoids observée est contemporaine de l’utilisation systématique, par les médecins de PMI, des courbes de corpulence éditées par le Programme national nutrition santé (PNNS). La sensibilisation des professionnels de santé au dépistage du surpoids des très jeunes enfants, associée aux campagnes de communication de grande ampleur pourrait constituer un outil efficace dans la prévention de l’obésité. Les populations à risque ont besoin d’être guidées avec des repères simples sans contrainte ni culpabilité pour limiter leur stigmatisation.

Contexte : Le surpoids et l'obésité infantile sont un enjeu de santé publique majeur du 21e siècle. Dans le carnet de santé, le tracé et l'analyse des courbes de corpulence sont un outil simple et fiable pour dépister les signes d'alerte précoces du risque de surpoids. La prévention et la prise en charge précoce des enfants doit permettre la diminution de la prévalence du surpoids. Objectifs : Evaluer le tracé des courbes de corpulence par les médecins qui suivent les enfants. Préciser quelle est l'estimation par les parents de la corpulence de leur enfant. Méthode : Analyse des courbes de corpulence antérieures puis complétées selon référence française et IOTF, dans les carnets de santé de 36 patients âgés de 3 à 17 ans, hospitalisés en Pédiatrie à l'Hopital d'Albertville. Questionnaire aux parents. Résultats : Aucune courbe de corpulence n'était remplie selon les strictes recommandations de la HAS de 2011. 25% étaient vierges. 75% avaient un nombre de points d' IMC insuffisant pour l'âge, dont 58,3% non reliés entre eux. Deux tiers des enfants en surpoids avaient été alertés par leur médecin. Presque 20% des parents avaient une mauvaise perception de la corpulence de leur enfant. Conclusion : Les courbes de corpulence sont mal exploitées par les médecins généralistes et pédiatres responsables du suivi de l'enfant. Les parents doivent être informés de la corpulence de leur enfant pour s'en rendre compte. La courbe de corpulence peut être une base objective pour échanger au sujet du surpoids. Le carnet de santé doit être réactualisé pour donner au médecin le meilleur outil de dépistage précoce, de diagnostic et de communication sur le surpoids de l'enfant.
