La conciliation des traitements médicamenteux est un moyen de sécurisation de la prise en charge médicamenteuse au transfert des patients, notamment dans le cadre d'un Groupement Hospitalier de Territoire (GHT). Une Analyse des Modes de Défaillances et de leurs Effets (AMDE) ainsi qu'une analyse de risques a posteriori a été réalisée pour identifier les points critiques dans la prise en charge médicamenteuse des patients transférés du Centre Hospitalier d'Avranches-Granville (CHAG) vers les services de Soins de Suite et de Réadaptation (SSR) du Groupe Hospitalier Mont-Saint-Michel. Une conciliation médicamenteuse a été mise en place pour les patients orientés vers les SSR des Centres Hospitaliers de Mortain, Saint-Hilaire-du-Harcouët et Villedieu-les-Poêles. Parmi les 95 patients transférés pendant la période de l'étude, 29 patients ont été conciliés à l'entrée et à la sortie du CHAG. La conciliation de sortie a permis d'intercepter 11 divergences non intentionnelles, dont 6 concernaient des classes médicamenteuses telles que les antalgiques et les anticoagulants. Une telle activité est difficile à développer en raison de son caractère chronophage, des difficultés d'identification des patients transférés et de la forte implication des professionnels de santé qu'elle nécessite. Cependant, elle permet de prévenir ou d'intercepter des erreurs médicamenteuses potentiellement graves, améliore la communication entre professionnels de santé dans l'hôpital, entre les centres hospitaliers du GHT et avec les professionnels de santé de ville.
