INTRODUCTION : En France, entre 35 et 50% des patients déments seraient dépressifs. Seulement 15 à 50% de ces personnes dépressives seraient diagnostiquées. Face au vieillissement de la population et donc à l'augmentation du nombre de patients déments à l'avenir, il serait intéressant d'améliorer le dépistage de la dépression. L'objectif principal de cette étude est de montrer l'intérêt d'utiliser un test de dépistage de la dépression chez les patients déments par un outil validé. METHODES : Nous avons mené une étude comparative, longitudinale, mono-centrique, avec un recrutement dans un CEG. Nous avons comparé les observations des gériatres n'utilisant pas de test validé, avec le résultat de l'échelle de Cornell, considérée comme le Gold-Standard. Cette échelle, a été remplie en interrogeant l'aidant principal, par contact téléphonique. Elle est validée pour le dépistage de la dépression chez les personnes démentes. Elle comprend 19 questions, elles-mêmes regroupées en 5 sous parties. RESULTATS : Cinquante-deux patients ont été inclus entre le premier juillet et le dix décembre 2014. Une plus grande sensibilité a été retrouvée par l'échelle de Cornell : 60% des patients étaient diagnostiqués comme dépressifs contre 23% par la clinique. Il n'apparaissait pas de profil dépressif particulier dans la population diagnostiquée comme dépressive par la Cornell, entre ceux diagnostiqués par le clinicien, et ceux qui ne l'étaient pas. CONCLUSION : Cette étude montre l'importance d'utiliser un outil validé pour dépister le syndrome dépressif dans une population démente. L'échelle de Cornell, pourrait être une aide précieuse pour améliorer ce dépistage en médecine générale.
