Contexte : la non-observance représente une barrière majeure à l'efficacité des soins dans les maladies chroniques. But de l'étude : déterminer l'implication de la perception du temps dans la genèse de l'observance médicamenteuse dans le diabète de type 2. Méthodes : étude quantitative utilisant un questionnaire administré à 120 patients présentant un diabète de type 2 hospitalisés dans le service d'endocrinologie de l'Hôpital Avicennes, hôpital universitaire APHP de la Seine Saint Denis. Résultats : Dans une analyse multivariée, seuls deux facteurs restaient significativement associés à l'observance médicamenteuse : une réponse impatiente dans un choix monétaire fictif, préférer recevoir 500 € aujourd'hui que 1500 € dans un an (RR 0,20 [CI 0,08-0,51], p=0,0006) et un long horizon temporel, défini par l'événement le plus tardif que les patients peuvent imaginer (RR 1,17 [CI 1,05-1,31], p = 0,006). Un horizon temporal long était aussi associé à un meilleur contrôle métabolique (RR 0,92 [CI 0,84-0,99], p = 0,044). Une analyse en correspondances multiples des données a montré l'implication, dans la genèse d'un comportement observant / non-observant, de trois dimensions de l perception du temps : l'horizon temporel, la patience / impatience, et la manière dont on se prévoit physiquement dans l'avenir. Conclusion : ces résultats, qui nécessitent d'être confirmés dans une étude multicentrique, suggèrent que chez les personnes atteintes de diabète, la perception du temps pourrait être un déterminant majeur de l'observance médicamenteuse et du contrôle métabolique.
