Emergency department (ED) overcrowding is a global, complex issue that occurs when the demand for emergency services surpasses patient care resources in an ED or hospital (George and Evridiki, 2015). It is characterized by decreased patient flow and adversely impacts patient safety, quality of care, and outcomes including increasing mortality (Carter, Pouch, and Larson, 2014) (Elder, Johnston, and Crilly, 2015). Many strategies to combat ED overcrowding aim to increase patient flow. Numerous studies have found that adding a physician or physician assistant (PA) in triage increases patient flow (Cheng and others, 2013) (Rowe and others, 2011) (Barbee, 2010). This process allows for earlier evaluation, intervention implementation, and diagnosis. Disposition decisions can even be made from triage. Specifically, this intervention has been shown to reduce ED length of stay (LOS), wait times, delays in treatment, and the number of patients who leave without being seen (Barbee, 2010) (Han and others, 2010). Study results include level 1 evidence showing a decrease in ED LOS and wait times when this intervention is implemented (Rowe, and others, 2011). Physician/PA triage is supported by the systems theory and a review of literature comprised of 15 peer-reviewed journal articles. Promoting the successfulness of physician/PA triage requires developing sound implementation, evaluation, and dissemination plans. The implementation plan presented describes how to gain approval and support from key stakeholders for the process change and includes a description of implementation logistics and required resources. The evaluation plan presented describes how to monitor and evaluate the effectiveness of physician/PA triage in improving patient flow. This includes the identification of evaluation methods, variables to be assessed, and tools. Additionally, an example of an educational tool is provided. Lastly, the dissemination plan describes how the results of the process change will be communicated to key stakeholders and the greater nursing community.

ABSTRACT IMPROVING EMERGENCY DEPARTMENT PATIENT FLOW THROUGH NEAR REAL-TIME ANALYTICS This dissertation research investigates opportunities for developing effective decision support models that exploit near real-time (NRT) information to enhance the "operational intelligence" within hospital Emergency Departments (ED). Approaching from a systems engineering perspective, the study proposes a novel decision support framework for streamlining ED patient flow that employs machine learning, statistical and operations research methods to facilitate its operationalization. ED crowding has become the subject of significant public and academic attention, and it is known to cause a number of adverse outcomes to the patients, ED staff as well as hospital revenues. Despite many efforts to investigate the causes, consequences and interventions for ED overcrowding in the past two decades, scientific knowledge remains limited in regards to strategies and pragmatic approaches that actually improve patient flow in EDs. Motivated by the gaps in research, we develop a near real-time triage decision support system to reduce ED boarding and improve ED patient flow. The proposed system is a novel variant of a newsvendor modeling framework that integrates patient admission probability prediction within a proactive ward-bed reservation system to improve the effectiveness of bed coordination efforts and reduce boarding times for ED patients along with the resulting costs. Specifically, we propose a cost-sensitive bed reservation policy that recommends optimal bed reservation times for patients right during triage. The policy relies on classifiers that estimate the probability that the ED patient will be admitted using the patient information collected and readily available at triage or right after.
