Les parodontopathogènes, sont le facteur étiologique majeur des parodontites. La destruction du tissu de soutien, est quant à elle due à la réponse de l'hôte à l'agression bactérienne. La réhabilitation implantaire des édentements partiels et complets est bien acceptée malgré des échecs. L'ostéointégration des implants peut-être compromise par la présence de bactéries intra-orales et des réactions inflammatoires concomitantes. Les lésions péri-implantaires sont des lésions inflammatoires qui peuvent affecter seulement la muqueuse (mucosite) ou aussi détruire le support osseux (péri-implantite). Les protocoles de traitement de la parodontite sont bien établis, alors que les différents outils thérapeutiques à disposition pour traiter la péri-implantite n'ont pas prouvé d'efficacité supérieure pour l'une ou l'autre technique. En plus des traitements conventionnels (mécaniques et chimiques), l'utilisation du laser est proposée pour le traitement de la parodontite et de la péri-implantite. De nombreuses études suggèrent que les échecs implantaires associés à une perte osseuse sont plus importants chez les sujets ayant une histoire de parodontite. Les implants montrant des signes de péri-implantite hébergent une flore similaire comparée à celles des parodontites. Les poches parodontales résiduelles sont un facteur de risque pour la péri-implantite, chez les patients traités pour une parodontite. L'élimination de ces parodontopathogènes de la cavité buccale avant la pose d'implants peut empêcher la colonisation bactérienne, et réduire le risque de péri-implantite. C'est donc le maintien de la santé parodontale plutôt que l'histoire de parodontite, qui semble être le déterminant principal dans l'apparition de la péri-implantite. Cependant, d'autres études prospectives contrôlées, à long terme et avec un échantillon suffisant de patients doivent être faites avant de tirer des conclusions définitives sur la survie implantaire chez les patients au parodonte compromis.
