Le cancer de l'enfant est une cause importante de mortalité infantile, mais la survie à long terme s'est considérablement améliorée ces dernières années. L'objectif de notre étude était d'évaluer l'impact des disparités socioéconomiques sur le délai diagnostic dans les cancers de l'enfant, ainsi que les conséquences sur la prise en charge du cancer et le pronostic. Nous avons réalisé une étude rétrospective multicentrique, basée sur la base de données RECAPGO. Au total, 854 enfants ont été inclus. Le délai médian de diagnostic était de 28 jours [12;64]. Le parcours de soins a un impact sur le délai de diagnostic : il augmente de manière significative lorsque les cliniciens inhabituels (ou autres) étaient impliqués en première intention, et diminue de manière significative lorsque les enfants étaient vus par un pédiatre ou aux urgences pédiatriques. Aucune des variables écologiques n'a été associée à un impact significatif sur le délai diagnostic. Cependant, à l'échelle individuelle, nous avons trouvé une tendance (p=0,061) pour les enfants de familles monoparentales, qui présentaient un délai diagnostic plus long par rapport aux autres. Les enfants atteints d'hémopathie et présentant des symptômes sévères avaient un délai diagnostic plus court par rapport aux enfants atteints des mêmes maladies. Dans la présente étude, le statut socio-économique semble avoir peu d'impact sur le délai diagnostic, tant à l'échelle individuelle qu'écologique, mais nous avons mis en évidence quelques pistes de réflexions à explorer.

In the United States, some populations suffer from far greater disparities in health than others. Those disparities are caused not only by fundamental differences in health status across segments of the population, but also because of inequities in factors that impact health status, so-called determinants of health. Determinants of health - like physical activity levels and living conditions - have traditionally been the concern of public health and have not been linked closely to clinical practice. However, if standardized social and behavioral data can be incorporated into patient electronic health records (EHRs), those data can provide crucial information about factors that influence health and the effectiveness of treatment. Such information is useful for diagnosis, treatment choices, policy, health care system design, and innovations to improve health outcomes and reduce health care costs. Capturing Social and Behavioral Domains and Measures in Electronic Health Records: Phase 2 identifies domains and measures that capture the social determinants of health to inform the development of recommendations for the meaningful use of EHRs. This report is the second part of a two-part study. The Phase 1 report identified 17 domains for inclusion in EHRs. This report pinpoints 12 measures related to 11 of the initial domains and considers the implications of incorporating them into all EHRs. This book includes three chapters from the Phase 1 report in addition to the new Phase 2 material. Standardized use of EHRs that include social and behavioral domains could provide better patient care, improve population health, and enable more informative research. The recommendations of Capturing Social and Behavioral Domains and Measures in Electronic Health Records: Phase 2 will provide valuable information on which to base problem identification, clinical diagnoses, patient treatment, outcomes assessment, and population health measurement.

Many Americans believe that people who lack health insurance somehow get the care they really need. Care Without Coverage examines the real consequences for adults who lack health insurance. The study presents findings in the areas of prevention and screening, cancer, chronic illness, hospital-based care, and general health status. The committee looked at the consequences of being uninsured for people suffering from cancer, diabetes, HIV infection and AIDS, heart and kidney disease, mental illness, traumatic injuries, and heart attacks. It focused on the roughly 30 million-one in seven-working-age Americans without health insurance. This group does not include the population over 65 that is covered by Medicare or the nearly 10 million children who are uninsured in this country. The main findings of the report are that working-age Americans without health insurance are more likely to receive too little medical care and receive it too late; be sicker and die sooner; and receive poorer care when they are in the hospital, even for acute situations like a motor vehicle crash.
