La mucoviscidose est une maladie génétique chronique grave qui évolue généralement de façon défavorable. Elle se caractérise par un épaississement des sécrétions muqueuses qui s'accumulent au niveau de nombreux organes entraînant ainsi de nombreuses co-morbidités. L'atteinte de l'appareil respiratoire est la principale cause de morbidité et de mortalité pour cette population L'objectif de ce travail est d'étudier l'impact que peuvent avoir les infections virales respiratoires sur des jeunes patients atteints de la mucoviscidose. Dans une première partie, nous avons décrit la mucoviscidose avec son épidémiologie, son mode de transmission, sa génétique, sa physiopathologie, les symptômes qu'elle induit, les différentes techniques de dépistage et diagnostique et enfin sa prise en charge. Dans une deuxième partie, nous avons abordé les virus respiratoires en détaillant leurs caractéristiques structurales, les signes cliniques qu'ils peuvent causer, les modalités de transmission, les méthodes de diagnostique, et enfin nous avons abordé la partie prophylaxie et traitement contre chacun de ces virus respiratoires. Dans une troisième partie nous avons expliqué les répercussions que peuvent entraîner des viroses respiratoires chez ce type de patients. Nous avons dans un premier temps abordé le rôle direct des virus sur les fonctions pulmonaires. En effet, ceux-ci auraient tendance via divers mécanismes à entraîner une inflammation exagérée et persistante conduisant à une aggravation des symptômes et des lésions pulmonaires. Ensuite, nous avons évoqué leur rôle indirect par interaction avec des bactéries. Les infections virales respiratoires favoriseraient la colonisation et les surinfections bactériennes, notamment chroniques à Pseudomonas aeruginosa, Haemophilus injluenzae et Staphylococcus aureus. Les infections virales respiratoires augmenteraient alors la morbidité et la mortalité des patients atteints de mucoviscidose. Enfin dans une dernière partie, nous avons réalisé une étude visant à évaluer l'incidence des viroses respiratoires chez de jeunes patients atteints de mucoviscidose et suivi au CRCM de Lyon. Nous avons comparé cette population à une population témoin non atteinte de la mucoviscidose afin de pouvoir déterminer si cette maladie représente un réel facteur de susceptibilité face aux infections virales respiratoires

Contexte : L'infection respiratoire à Achromobacter species chez les patients atteints de mucoviscidose est de plus en plus fréquente. L'impact de ce germe sur l'aggravation de la maladie reste controversé. Nous avons donc évalué le retentissement respiratoire à court et moyen terme de cette infection. Méthode : Etude cas-témoin observationnelle rétrospective monocentrique au CRCM pédiatrique de Lille. Nous avons inclus 20 patients dont au moins un des examens cytobactériologiques des crachats était positif à Achromobacter species, de 2014 à 2018. La sélection des témoins était réalisée grâce à un appariement sur le sexe, l'âge et le portage de Pseudomonas aeruginosa. Nous relevions les données anamnestiques, le statut respiratoire (VEMS : volume expiratoire maximal seconde, exacerbations annuelles, co-infections) et l'IMC (indice de masse corporelle) un an avant, et jusqu'à 5 ans après l'infection. Résultats : La prévalence de l'infection à Achromobacter était de 13 % sur notre CRCM pédiatrique. La chronicisation de l'infection touchait 45 % de la population infectée. Une perte de 29,5 % du VEMS médian chez les cas était constatée à cinq ans de l'infection contre 9 % chez les témoins (p=0,01). Cette diminution était d'autant plus importante chez les cas chroniques avec un VEMS significativement plus bas à un an de 45 % comparativement aux témoins (p=0,004) et de 22 % comparativement aux cas intermittents (p=0,04). Dans le groupe chronique, trois enfants avaient au cours du suivi une transplantation pulmonaire et six avaient un soutien ventilatoire ou une oxygénothérapie longue durée. Conclusion : Notre étude est faveur d'une aggravation induite par l'infection à Achromobacter species d'autant plus marquée lorsqu'elle est chronique, chez les enfants atteints de mucoviscidose.

Introduction : les surinfections bactériennes respiratoires sont une des principales complications de la mucoviscidose. Parmi les germes fréquemment identifiées, Achromobacter xylosoxidans (A. xylosoxidans) est une bactérie dont la pathogénicité reste controversée. L’objectif était d’étudier l’impact de la primo-infection à A. xylosoxidans sur l’atteinte respiratoire d’enfants atteints de mucoviscidose et d’identifier des facteurs de risque. Méthodes : nous avons mené une étude rétrospective multicentrique descriptive puis cas-témoin. Les cas, définis par un Examen Cytobactériologique des Crachats positif à A. xylosoxidans, ont été appariés sur l’âge, le sexe et les mutations du gène CFTR, à des témoins jamais infectés. Les données cliniques ont été recueillies au moment de la primo-infection (T0), 1 et 2 ans avant (T-12 et T-24), 1 et 2 ans après (T+12 et T+24). Résultats : 45 cas ont été inclus (prévalence de la primo-infection : 10,6 %, âge moyen : 11,5 ans). La fonction respiratoire des cas, estimée par le VEMS (Volume Expiratoire Maximal en une Seconde), diminuait significativement après la primo-infection (p=0,04). Le delta du VEMS des cas entre T+12 et T-12 semblait avoir une tendance non significative à être plus important comparé au delta du VEMS des témoins, qui restait stable (-5,5% vs -0,5% respectivement, p=0,14). Les cas étaient des patients plus sévères que les témoins avec, à T-24 et T+24 respectivement, un plus grand nombre d’exacerbations (p=0,02 et p=0,0001), d’hospitalisations (p=0,05 et p=0,0001) et de cures d’antibiothérapies orales (p=0,001 et p=0,0001) et intra veineuses (p=0,03 et p=0,0001). Conclusion : cette étude cas-témoins montre que la primo-infection à A. xylosoxidans semble toucher les patients les plus sévères et pourrait être considéré comme un marqueur de sévérité de l’atteinte pulmonaire chez les patients atteints de mucoviscidose.

In 1969 the U.S. Air Force issued a statement that read' "No UFO reported, investigated and evaluated by the Air Force was ever an indication of threat to our national security." This statement is patently false.
