En France, les GEA infantiles sont un problème de santé publique qui nécessite d'améliorer les pratiques de réhydratation orale. Les effets bénéfiques de l'information écrite sur les connaissances des patients ont été démontrés dans certaines pathologies. Notre objectif principal était d'évaluer l'impact d'une fiche d'informations sur les connaissances des parents en cas de GEA de l'enfant. Etude prospective, randomisée, comparative et multicentrique, réalisée en Rhône-Alpes avec 15 médecins généralistes de janvier 2011 à septembre 2012. Déroulement : inclusion des parents d'enfants âgés de 3 à 36 mois atteints de GEA. Remise ou non de la fiche en fin de consultation. Recueil des données par questionnaire téléphonique 8 à 10 jours après la consultation. Analyse du score de connaissances sur 12 points et des objectifs secondaires. En France, la gastro-entérite aiguë (GEA) reste un problème de santé publique en termes de morbi¬mortalité et coûts sociétaux. Notre objectif principal était de décrire les pratiques des médecins généralistes parisiens dans la prise en charge de la GEA de l'enfant de moins de 6 ans durant l'hiver 2018-2019. Cette étude prospective, descriptive et analytique est basée sur 107 réponses recueillies par questionnaire. Des conduites excessives étaient identifiées dans les prescriptions de coprocultures et les orientations hospitalières. Le soluté de réhydratation orale n'était systématiquement prescrit que par 68 % des médecins, et les traitements adjuvants étaient largement retrouvés. Les prescripteurs d'antiémétiques (77,9 %) avaient principalement recours à la métopimazine (50,6 %). Des mesures nutritionnelles étaient parfois injustifiées, dont le régime anti-diarrhéique (92,5 %). L'information parentale était le plus souvent donnée par oral, mais 37,5 % des médecins n'informaient pas sur les indications à consulter lors du prochain épisode. Les médecins ne connaissaient pas les recommandations dans 79,4 % des cas, notamment du fait d'une diffusion insuffisante (62,2 %). Pour ceux qui les connaissaient, l'influence parentale pouvait en être un frein à leur application (77,2 %). Notre étude soulève des limites perfectibles. Les formations continues devraient être plus adaptées aux besoins ressentis. Pour l'éducation parentale, les médecins devraient s'appuyer sur l'utilisation de fiches conseils et du carnet de santé. Une Enquête de Pratiques Professionnelles en Ile-de-France s'envisage, comme l'étude de l'impact de l'outil de synthèse clinique créé dans le cadre de l'étude.

Having safe drinking water is important to all Americans. The Environmental Protection Agency's decision in the summer of 2001 to delay implementing a new, more stringent standard for the maximum allowable level for arsenic in drinking water generated a great deal of criticism and controversy. Ultimately at issue were newer data on arsenic beyond those that had been examined in a 1999 National Research Council report. EPA asked the National Research Council for an evaluation of the new data available. The committee's analyses and conclusions are presented in Arsenic in Drinking Water: 2001 Update. New epidemiological studies are critically evaluated, as are new experimental data that provide information on how and at what level arsenic in drinking water can lead to cancer. The report's findings are consistent with those of the 1999 report that found high risks of cancer at the previous federal standard of 50 parts per billion. In fact, the new report concludes that men and women who consume water containing 3 parts per billion of arsenic daily have about a 1 in 1,000 increased risk of developing bladder or lung cancer during their lifetime.
