But: Analyser l'impact de la prescription d'un traitement de prévention secondaire adapté au décours d'un IDM à la survenue d'évènements cliniques majeurs (mortalité, récidive d'IDM ou AVC) en fonction du niveau de risque cardiovasculaire défini par le score TRS-2P. Méthode: Nous avons utilisé le registre FAST-MI 2010 incluant 4,169 patients hospitalisés pour IDM. Trois niveaux de risque ont été défini par le score TRS-2P: groupe 1 (faible risque; TRS-2P≤1); groupe 2 (risque intermédiaire; TRS-2P=2), groupe 3 (haut risque; TRS-2P≥3). Le traitement de prévention secondaire adapté était défini selon les dernières recommandations (bi-antiagrégants et statines pour tous, IEC/ARA2 et béta bloquants quand indiqué). Résultats: La prévalence des groupes 1, 2 et 3 était de 46%, 25% et 29% avec un traitement de prévention secondaire adapté à la sortie chez respectivement 79%, 69% et 54% des patients. En analyse multivariée, la prescription du traitement adapté était associée à un taux d'évènements cardiovasculaires plus faible à 5 ans (HR=0.78; IC95%[0.68-0.91],P=0.01), notamment chez les patients à haut risque du groupe 3: HR=0.71 (IC95%[0.58-0.86], P=0.001). La même tendance a été trouvée pour les statines fortes doses et les nouveaux inhibiteurs du P2Y12. Conclusion: La prescription d'un traitement de prévention secondaire adapté à la sortie d'hospitalisation est inversement corrélée au risque des patients alors que ce sont ceux à haut risque qui en bénéficieraient le plus. Des efforts de prescription sont à fournir, surtout chez les patients à haut risque.
