The Trabecular Metal Implant is made with a midsection of Tantalium based biomaterial, that is 80% porous, with a trabecular structure designed for bone ongrowth and bone ingrowth .Pourpose:This retrospectiv study was conducted to evaluate the clinical efficency and radiological measurements of immediate placement and occlusal loading of single-tooth restorations on hybrid titanium dental implants with highly porous, unthreaded, tantalum-based midsections in fresh extraction sockets, positioned from January to December 2013.Materials and Methods :A retrospective review of patient records was conducted in a single private-practice setting at 1,2 and 4 years. Single, non restorable teeth were atraumatically extracted using an incision-free, flapless tecnique. Reasons for extraction included crown/root fractures, deep caries and/or endodontic failures. Implants were lingually placed 3-4 mm apically to the free gengival margin in fresh extraction sockets and the gap around the implants were grafted. The length of the implants ranged from 10-13 mm and the implant diameter ranged from 4.1 to 6.0 mm, A temporary crown screwed in acrilyc resin was immediately delivered and placed into occlusion. Two weeks later, implants were definitively restored with glass ceramic crowns. Results:Twenty-six patients were treated for one non-restorable tooth (n=26). Most implant were placed in maxillary (n=16/26) and premolar (n=17/26) locations, and in lower density (Type 3) bone (n=23/26). All implant insertion torque levels were >40 Ncm (n=26/26). Mean ISQ values were 71.6 immediately after placement. All implants were provisionalized immediately with screw-retained crows (n=26/26). Pink Esthetic Scores were 13.4 at 1 year and 13.0 at the 4-year follow-up. Mean peri-implant probing were 2,35 mm+- 2.4 at 1 year and 2,92 mm +- 2.6 at 4 years of follow-up. Mean crestal bone loss was 0.58 mm +- 0.34 at 1 year and 0,869 mm +- 0.36 at the 4-year follw-up. Implant survival and success rates were 100% (n=26/26), respectively, with a follow-up time of 4 years. There were no irresolvable complications. All patients reported full satisfaction for mastication function, phonetics, and esthetics.Conclusion:With the limits of this retrospective study, the use of Tantalum-based Implants in post-extractive sites with immediate provisional restoration turns out to be a safe procedure and predictable over the long term with stability of hard and soft tissues.

Resonance frequency analysis (RFA) and insertion torque (IT) has been proposed to assess this stability with a quantitative method.Aim of the Study: To investigate the clinical outcome, marginal bone maintenance and implant survival for implant treatment using the fast loading protocol which include immediate provisionalization and early delivery of the definitive prosthesis. Materials and Methods: 25 patients were treated with 48 Neoss ProActive Tapered implants placed in both maxilla and mandible in healed bone as well as in fresh extraction sockets. Minimum primary stability, (IT >35 ncm and RFA >60), for all single implants was established before subjected to immediate (within 48 h), non-functional, load using screw-retained temporary restorations (single or maximum 2 splinted implants with 2-3 teeth units). Definitive prostheses were delivered within 6-8 weeks from implant placement, based on RFA measurements during early healing phase. IT, RFA, intraoral radiographs, and impressions of implant positions were registered at implant placement. During clinical follow-up RFA was measured at 2, 4, 6 weeks and 6 months from implant placement. Marginal bone level measurements were performed from intraoral radiographs taken at implant placement, 6-, and 12-month follow-up visits.Results: 48 implants were subjected to the fast loading protocol. IT was over 40 Ncm for 46 implants. Two implants with IT35 Ncm were both splinted to another implant with IT50 Ncm, tightening the retention screw with low forces. No implants were lost during the observation period, giving an implant survival rate of 100%. Mean RFA measurements remained stable without any decrease during the initial healing phase, 78.3u00b14.0 in the mesiodistal direction and 77.6u00b14.4 in the buccolingual direction at placement and 81.3u00b13.7 and 80.8u00b13.6 respectively at 6 months. Radiographic readings showed that at placement the implant platform was at bone level (+0.13 mm). Mean marginal bone remodeling was -0.47u00b10.38 mm from implant placement to 1 year, -0.49u00b10.38 mm from implant placement to 6 months, and only 0.01u00b10.28 mm between 6 to 12 months follow-up. No significant differences could be found for marginal bone remodeling between implants placed in healed bone and those placed in fresh extraction sockets.Conclusions: Achieving good primary stability (IT of 40 Ncm) with maintained implant stability during initial healing phase can guarantee predictable clinical outcome for patients subjected to a fast loading protocol. Fast loading showed no significant differences in marginal bone loss in the first year of follow-up as compared to other studies investigating the same implant type but followed different surgical and prosthetic protocols.

Endosseous dental implants have been successfully utilized for the last 3 decades to restore partially and fully edentulous patients with at least 3 months healing period of the alveolar bone following tooth extraction. An increasing number of clinical publications report on immediate implant restoration in the extraction. The advantages of immediate implantation after tooth extraction are shorter treatment time, fewer surgical procedures, and ensuring optimal tooth tissue aesthetics. Frequently, however, compromised teeth that are indicated for surgery are involved with acute and chronic infectious conditions which traditionally contraindicate their immediate restoration with dental implants. A review of current literature suggests that immediate implantation in infected sites should be avoided. Recent experimental studies have corroborated this clinical experience and shown that professional alveolar socket debridement and broad-spectrum antibiotics generate adequate conditions for the bone remodelling process around immediate dental implants placed into infected sites.Background: The immediate placement of dental implants into fresh non-infected sockets is an accepted successful technique, while placement of these implants into pathological Dento-alveolar sockets is still a controversial issue.Aim/Hypothesis: The aim of this clinical case-serious study was to report, retrospectively, the survival rate of immediate dental implants placed immediately after extraction of teeth with infected Dento-alveolar sockets in the maxilla and mandible at the osseointegration period (3 months).Materials and Methods: A retrospective analysis was made of the clinical and radiographic findings corresponding to 15 consecutive patients (7 women and 8 men, between the ages 21and 59 years) with a mean age of 44.14 years (SD= 9.28). A total of 36 locking-taper Bicon dental implants (hydroxyapatite, HA-coated) were immediately placed into infected fresh extraction- sockets included radicular cysts, chronic peri-apical lesions and sub-acute periodontal and endodontic infections using the flap or flapless surgery. Retrospective data on implant position, implant size, pathological condition, and baseline radiographs were obtained from patient records. Clinical and radiographic analyses were obtained to evaluate the treatment-outcome. Data were analyzed using descriptive statistics.Results: All the implants (N=36) except 2 achieved osseointegration in 3 months follow up. This reflects a 94.44% success rate within this small study group and 5.55% failure rate. Complications were related to the reasons for tooth loss.Conclusions: This retrospective clinical case-serious study showed the possibility thatimmediate dental implant placement might be successful in infected extraction sockets.
