La GEU voit depuis quelques années son incidence se stabiliser mais elle reste la première cause de mortalité des femmes pendant le premier trimestre de la grossesse. Sa morbidité immédiate est liée au risque de rupture tubaire. Aujourd’hui son traitement de référence est la coelio-chirurgie mais le traitement médical par administration de MTX en dose unique par voie intra-musculaire est une alternative non négligeable car aussi efficace, peu invasive, peu iatrogène et peu coûteuse. Ses indications restent limitées et font le sujet des débats actuels sur la prise en charge de la GEU. Nous avons réalisé une étude rétrospective comparative des deux méthodes de traitement. Elle porte sur 106 patientes âgées de 30 ans en moyenne, séparées en deux groupes : traitement par MTX et traitement coeliochirurgical conservateur. Les facteurs de risque retrouvés sont les antécédents de chirurgie abdomino-pelvienne et le tabagisme. Le diagnostic a été porté à un terme moyen de 6SA dans le groupe MTX et 7SA dans le groupe chirurgie, sur des arguments cliniques, biologiques (stagnation des BHCG) et surtout échographiques (masse latéro-utérine visible chez 73% des patientes du groupe MTX et 90% des patientes du groupe chirurgie ; hémopéritoine plus ou moins abondant.). Notre taux d’efficacité du traitement médical est de 83% et celui du traitement chirurgical de 93,5% ce qui est comparable aux données de la littérature (traitement médical 65 à 95% et traitement chirurgical 72 à 95%). Le traitement médical a été très bien toléré. La fertilité ultérieure est, dans notre étude comme dans la littérature, un point difficile à étudier compte tenu du faible nombre de patientes qui reviennent en consultation de suivi. Mais il semble que le taux de grossesses ultérieures soit le même pour les deux méthodes Le facteur conditionnant le plus la fertilité ultérieure étant surtout l’âge et les autres facteurs d’infertilité associés. Le traitement médical présente aussi l’avantage d’être peu coûteux et partiellement réalisable en ambulatoire, permettant de diminuer considérablement le coût de la prise en charge de la GEU.

The mechanism of action, safety, and efficacy of IUDs were reviewed by a WHO Scientific Group in 1986. The Scientific Group concluded that the IUD should continue to be supported, in both developed and developing countries, as a safe, reliable method of fertility regulation. The newer copper-releasing devices are comparable to oral contraceptives in terms of safety and efficacy. When compared to women who use other reversible methods of contraception, IUD users have the lowest mortality resulting from deaths directly attributable to those methods or to the consequences of unwanted pregnancy. In the past decade, research has concentrated on the development of new devices that have both higher continuation rates and lower rates of expulsion and removal for bleeding abnormalities. An important recent concern has been the possible increased risk of pelvic inflammatory disease (PID) and subsequent tubal infertility associated with IUD use. However, it now appears that methodological problems have caused the IUD-associated risk of PID to be overestimated. The increased risk with IUDs seems to be limited to the 1st 4 months of use. No increased risk of tubal infertility has been found among IUD users in stable, monogamous sexual relationships. The use of a copper IUD after the 1st pregnancy is not associated with secondary infertility due to tubal disease. Finally, the newer copper IUDs have low rates of ectopic pregnancy.
