The analysis aims to describe processes of demographic and epidemiological change, as well as health status self-assessment in selected Central and Eastern Europe countries (CEECs), including both the new member states and the candidate countries. The analysis is presented in the context of the use of medical services and the structure of services used. Special attention is given to those demographic and epidemiological changes that have direct impact on the frequency of medical services used and, as such, determine increases in health care costs. The ageing process and health status improvement are the main hypothethical determinants of health care costs and are therefore presented in more detail. Additionally, changes in health behaviour - mainly in the use of medical services - are discussed in the context of institutional changes in the health care sector. Comparative analysis covers the countries representing groups characterised by similar tendencies and specific health and demographic characteristics. Estonia respresents the Baltic states, Bulgaria, the Balkan countries, and Slovakia represents countries of Central Europe. Poland is a specific country, with demographic and epidemiological characteristics similar to Slovakia, but it is much larger, with a high share of rural population. In Hungary, demographic processes related to the second demographic transition began much earlier and are still dynamic, and as such, constitute a reference for other CEECs.

Reform of the welfare sector is an important yet difficult challenge for all countries in transition from socialist central planning to market-oriented democracies. Here a scholar of the economics of socialism and post-socialist transition and a health economist take on this challenge. This 2001 book offers health sector reform recommendations for ten countries of Eastern Europe, drawn consistently from a set of explicit guiding principles. After discussing sector-specific characteristics, lessons of international experience, and the main set of initial conditions, the authors advocate reforms based on organized public financing for basic care, private financing for supplementary care, pluralistic delivery of services, and managed competition. Policymakers need to achieve a balance, both assuring social solidarity through universal access to basic health services and expanding individual choice and responsibility through voluntary supplemental insurance. The authors also consider the problems that undermine effectiveness of market-based competition in the health sector.

Good health can be considered one of the most fundamental resources for social and economic prosperity. While the goal to improve average levels of population health is important, there has been an increasing focus on disparities at national and European levels. Improvements have been seen over the past few decades in both health status and living and working conditions has widened tremendously in the European Union (EU) and will continue to do so as it goes through the enlargement process. The diversity in living conditions has translated into diversity in patterns of health across the region. Inequalities in income, education, housing and employment affect population health, both directly (for example, good housing reduces risks associated with poor health) and indirectly through psychosocial factors (such as stress). From the life course perspective, individuals are affected by different sets of risks related to disease and illness; certain diseases and causes of health are more likely to affect young people, whereas the majority are associated with older ages. Investigating differences in health status within and between European countries provides the focus of this report. The relationship between living conditions, socioeconomic factors and health is discussed and analysed with the objective of stimulating a debate and policy action for creating a healthier and more equitable society. We aim to present an overview of key issues and not comprehensive literature review or exhaustive analysis of the topics involved.
