This timely contribution to the global literature on health inequities approaches the subject through a synthesis and analysis of relevant published literature on India. Amongst the BRICS countries, India ranks the lowest in the gender-gap index and has the highest poverty rate, and there is clear evidence that socio-economic inequalities have increased in India in the twenty-first century. These have direct impact on the health conditions of its people; however, there has been relatively little concerted research attention on health inequities in India. This volume fills the gap by synthesizing research evidence since the year 2000 on the topic. This is perhaps the first volume on this topic of such scope and breadth. Its uniqueness lies in the synthesis of evidence across a range of axes of disadvantages within a single volume: socio-economic position, caste, gender, other socially constructed vulnerabilities such as disability, HIV status, migrant status; and health-system factors contributing to or mitigating inequities in health. Each core chapter not only summarizes research findings but also engages critically with the perspectives reflected in the chapters and proposes a framework for understanding the mechanisms through which health inequities result. The WHO European Region has seen remarkable health gains, though inequities persist both between and within countries. Much more is understood now about the extent and social causes of these inequities, particularly since the 2008 report of the Commission on Social Determinants of Health. This review of inequities in health across the 53 Member States of the Region was commissioned to support the development of the new European policy framework for health and well-being, Health 2020. It builds on the global evidence and recommends policies to reduce health inequities and the health divide across all countries, including those with low incomes. The report is presented in four parts. Part I provides the context and background to the review, and sets out the key principles underpinning the recommendations and the rationale for grouping them into four broad themes: life-course stages, wider society, the broader macro-level context, and governance, delivery and monitoring systems. Part II summarizes current evidence on the magnitude of the health divide among European Region countries, describing the inequities in health and their social determinants. Part III focuses on the four themes, making recommendations with supporting evidence. Part IV outlines the implementation issues, summarizes the framework for action, discusses reasons for failure, provides guidance on good practice and summarizes the review's conclusions and recommendations. "This paper describes approaches to the measurement and explanation of income-related inequality and inequity in health care financing, health care utilization and health and considers the applicability and the feasibility of these methods in low-income countries. Results from a comparative study of 14 Asian countries are used to illustrate the main issues. The structure of health finance in low-income countries, in particular the heavy reliance on out-of-pocket payments, means that the equity issues in finance are quite different from those of concern in high-income countries. Primary concern is not with the distribution of contributions to pre-payment mechanisms but with the deterrent effect of payments on utilization and the distribution of uninsured payment risks. Measurement of inequity in utilization of health care in low-income countries is constrained by the lack of reliable measures of health that can be used to standardize..." -- abstract.
