Over the past thirty years, firefighting equipment, procedures, and training have improved, and the overall U.S. firefighter death rate has declined. However, in the category of traumatic injury on the fireground, the U.S. firefighter fatality rate is increasing. The problem was that it was not clear why this is so. The purpose of the present research was to test a set of higher-order causes, also known as root causes, which were derived from firefighter fatality case studies, against recent firefighter fatality case studies in order to evaluate the usefulness of the higher-order causes in understanding the U.S. firefighter line-of-duty-death (LODD) problem. Evaluative research was used to answer the questions: Does the set of higher-order causes apply to recent LODD events? And, how could the set of higher-order causes be used to improve firefighter safety? This research first looked at the distribution of Fire Fighter Fatality Investigation Prevention Program (FFFIPP) recommendations for medical incidents and vehicle℗Ư related trauma incidents in order to check some conclusions on which the root cause analysis was based. The applicability of each of the four higher-order causes was then evaluated for all trauma incidents. Although the distribution of vehicle-related FFFIPP recommendations differed from that on which the root cause analysis was based, the set of higher-order causes was found to be applicable to all of the incidents studied. Four recommendations were developed: use the Four Higher-Order Causes as a training tool, strive to replace an ideal of sacrifice with the values of competence and devotion to duty, fireground heat injury prevention, and redefine "vehicle℗Ư related" LODD incidents in future FFFIPP studies.

The National Institute for Occupational Safety and Health (NIOSH), an institute within the Centers for Disease Control and Prevention (CDC), is the federal agency responsible for conducting research and making recommendations for the prevention of work-related injury and illness. In fiscal year 1998, the Congress appropriated funds to NIOSH to conduct a fire fighter initiative. NIOSH initiated the Fire Fighter Fatality Investigation and Prevention Program to examine deaths of fire fighters in the line of duty so that fire departments, fire fighters, fire service organizations, safety experts and researchers could learn from these incidents. The primary goal of these investigations is for NIOSH to make recommendations to prevent similar occurrences. These NIOSH investigations are intended to reduce or prevent future fire fighter deaths and are completely separate from the rulemaking, enforcement and inspection activities of any other federal or state agency. The NIOSH Fire Fighter Fatality Investigation and Prevention Program conducts investigations of fire fighter line-of-duty deaths and injuries to formulate recommendations for preventing future deaths and injuries. The program does not seek to determine fault or place blame on fire departments or individual fire fighters but to learn from these tragic events and prevent future similar events. The Program Objectives are: Better define the characteristics of line-of-duty deaths among fire fighters; Develop recommendations for the prevention of deaths and injuries; Disseminate prevention strategies to the fire service. The enclosed investigation reports are examples of reports for traumatic injury and cardiovascular disease deaths of fire fighters in the last couple of years. Under its program, NIOSH investigators interview persons with knowledge of the incident and review available records to develop a description of the conditions and circumstances leading to the deaths in order to provide a context for the agency's recommendations.

The overall goal of the NIOSH FFFIPP is to reduce the number of fire fighter fatalities. To accomplish this goal, NIOSH conducts investigations of line-of-duty fire fighter deaths to identify contributing factors and to generate recommendations for prevention. This document summarizes the most frequent recommendations from the first 8 years of the NIOSH Fire Fighter Fatality Investigation and Prevention Program (FFFIPP). The overall goal of the program is to reduce the number of fire fighter fatalities. Through 2005, the FFFIPP investigated 335 fatal incidents involving 372 fire fighter fatalities. The investigations encompassed a variety of circumstances such as cardiovascular-related deaths, motor vehicle accidents, structure fires, diving incidents, and electrocutions. Fatalities have been investigated in career, volunteer, and combination departments in both urban and rural settings throughout the United States. This document shares the most common recommendations from the 335 investigations and more than 1,286 recommendations that were developed by NIOSH investigators. These recommendations were developed using existing fire service standards, guidelines, standard operating procedures, and other relevant resources over the first eight years of the program. Fire departments can use this document when developing, updating, and implementing policies, programs, and training for fire fighter injury prevention efforts.
