Introduction : les fractures articulaires comminutives déplacées du radius distal sont les plus délicates à traiter, l'arsenal est vaste, le traitement non consensuel. L'objectif de cette étude était d'évaluer les résultats cliniques et radiographiques de ces patients, et de les comparer selon le mode d'ostéosynthèse employé et selon l'âge. Matériel et méthodes : il a été réalisé une étude monocentrique rétrospective sur une série de 109 patients avec un suivi moyen de 18 mois. Résultats : les résultats cliniques et radiographiques de la série entière étaient satisfaisants, malgré un taux important de complications du fait de la complexité de ces fractures. Les résultats des patients traités par fixateur externe étaient inférieurs aux résultats des patients traités par plaque ou par plaque associée à des broches, qui étaient eux comparables. Les résultats cliniques des patients de 65 ans et plus étaient semblables aux autres, sans différence de traitement, même si les résultats radiologiques tendaient à être moins bons. Conclusion : malgré le caractère articulaire et la comminution de ces fractures, les résultats sont corrects quel que soit l'âge. Il faut bien peser l'indication du fixateur externe qui est à réserver aux fractures C3 très comminutives nécessitant une greffe, et penser à l'alternative prothétique en cas de fracture irréparable du sujet âgé autonome.

The second English-language edition of the Small Fragment Set Manual was enthusiastically received and quickly went into a second printing. In preparing a third edition, we found it necessary to revise the text extensively and partly restructure it. The reasons for this are numerous. Experience of recent years has brought technical refinements in the operative treatment of many types of small fracture. Many of these changes stem from the small-fragment-set training programs conducted in Switzerland since 1980, and also from courses and symposia that have been held in other European countries and the United States. These events were occasions for a fruitful ex change of experience with surgeons who were critical of our methods. Pathological conditions affecting the hip and knee joints occupy a particular place amongst the important orthopaedic entities affecting the extremities. On the one hand they are relatively frequent and on the other they mean for the patient limitation of his ability to walk, because of their considerably detrimental effects. A purposeful basic treatment of these joint diseases (and here osteoarthritis takes pride of place) is only possible if it stems from a reliable biomechanical analysis of the normal and patho logical stressing of the joint in question. Whilst the situation in the hip can be considered to be fundamentally clarified, a com prehensive representation of the knee is still lacking particularly when taking into account the latest knowledge of biomechanics. Recently our concepts of the kinematics of the knee have been completely changed, but the clinically important question of articular stressing remains unanswered. Dr. Maquet has carried out pioneer work in this field for some years in adapting, by analogy, to the knee joint, principles already accepted for the hip joint. Since the knee is not a ball and socket joint, a complicated problem arises for which new thoughts are necessary. The results of the numerous operations carried out by Dr. Maquet according to the biomechanical con siderations, demonstrate that his thinking is fundamentally correct.
