The symposium "Family Dynamics, Family Therapy, and Pediatric Medical Illness," held at Downstate Medical Center on December 12 and 13, 1980, considered the impact of life-threatening illness in children and adolescents on intrafamilial dynamics. A group of experts addressed the practical and theoretical psychological and social issues facing pediatric patients and their families when con fronting chronic and severe childhood illnesses including brain damage, cancer, hemophilia, juvenile diabetes, and heart disease. The presentations and group discussions clearly revealed the complexity of physical and psychological problems posed by the seriously ill child with chronic disease for both the health care provider and the family. The conference proceedings confirm that quality care and treatment for the ill child requires the partici pation of a variety of health care disciplines representing diverse fields of knowledge. Any parent who has raised more than one child is likely to be keenly aware of subtle or even striking differences among their offspring. The central premise of this volume is that children bring personal qualities to their relationships with other family members that help shape family interaction, relationships, and even processes that family researchers have called "parenting." In the 1990s it is no longer "news" that families do not operate independently from other social organizations and institutions. Instead, it is generally recognized that families are embedded in a complex set of relationships with other institutions and contexts outside the family. In spite of this recognition, a great deal remains to be discovered about the ways in which families are influenced by these outside agencies or how families influence the functioning of children and adults in these extra-familial settings--school, work, day-care, or peer group contexts. Moreover, little is known about the nature of the processes that account for this mutual influence between families and other societal institutions and settings. The goal of this volume is to present examples from a series of ongoing research programs that are beginning to provide some tentative answers to these questions. This volume is the product of a combined effort to find programs of service delivery that demonstrably treat the varieties of mental health problems of children and their families. The Section on Clinical Child Psychology (APA, Clinical Psychology Section I) and the Division of Child, Youth, and Family Services (APA, Division 37) established a task force whose mission was to identify, provide recognition for, and disseminate information on such programs. Their findings are presented here. The opening chapter and each section overview chapter provide orientations to the program descriptions and examine characteristics of exemplary interventions. The targeted problems include: child abuse and neglect, school adjustment problems, social problem-solving problems, autism and developmental disabilities, conduct disorders and severe emotional problems, children affected by disasters and trauma, children whose parents are divorced, children of teenage parents, family dysfunction and parent-child relationships, oppositional defiant disorder, and attention deficit disorder. Settings for interventions in the model programs include: schools, mental health centers and family guidance clinics, hospitals and pediatric practices, group homes and sheltered workshops, community centers, family homes, summer camps, and coordinated systems of care. The 18 programs described demonstrate the rationale for their interventions, their targeted populations, the type of staff and personnel, various programmatic interventions, aspects of the problems, implementation of interventions, and how the programs have been evaluated. The needs for improved mental health services remain strong. The supporting organizations and the members of the Task Force intend for the product of this project to be helpful in providing models for meeting those needs.
