Introduction : face au contexte démographique, les soins premiers doivent s'approprier le concept de fragilité. Le projet "Frailty and Alzheimer's disease prevention into Primary care" (FAP), a pour objectif d'implémenter en soins premier une infirmière d'évaluation gérontologique ou un interne formé, pour évaluer les sujets repérés fragiles. Notre travail avait pour but d'évaluer le degré de satisfaction des patients ayant participé à ce projet. L'objectif secondaire était de comparer la satisfaction entre le groupe Infirmières et le groupe internes. Matériel et méthodes : il s'agit d'une étude observationnelle descriptive réalisée en région Occitanie sur les 271 patients évalués initialement dans le projet. Le recueil de données a été effectué par l'intermédiaire d'un questionnaire téléphonique. Résultats : 133 questionnaires ont pu être recueillis. 72% (n=89) des patients ont été complètement satisfaits de l'évaluation gériatrique. 42% (n=52) auraient eu tendance à refuser si l'évaluation leur avait été proposée à l'hôpital. 91% (n=31) des patients qui ont eu un plan personnalisé de soins en étaient plutôt satisfait. L'analyse des comparaisons ne permet pas de dégager de différences statistiquement significatives entre les deux groupes. Conclusion : le retour des patients est très encourageant. Ils se sont montrés favorables au développement des évaluations gériatriques en soins premiers et à la généralisation du projet "FAP"à d'autres territoires. De nouvelles études doivent être menées afin d'évaluer au niveau rnédico économique ce nouveau modèle de prise en charge de la personne âgée.

D'après l'OMS, on compte 47,5 millions de personnes atteintes de démence dans le monde et en raison d'une augmentation de l'espérance de vie, ce nombre croît de manière alarmante et est à l'origine d'un véritable problème de santé publique. La Haute Autorité de santé recommande dans le cadre d'un diagnostic précoce, une évaluation initiale qui correspond à l'évaluation gériatrique multidimensionnelle. Se pose donc la question de la faisabilité en médecine générale de cette évaluation gériatrique multidimensionnelle, reconnue comme étant le meilleur moyen de suivre une personne âgée dans sa globalité. Evaluer l'acceptation, la faisabilité et la fiabilité en médecine générale de l'évaluation gériatrique multidimensionnelle chez les patients déments à l'aide d'un questionnaire. Recrutement des 200 premiers patients de l'année 2014 consultant pour la première fois dans le centre d'évaluation gérontologique et de troubles de la mémoire du CHU de Clermont-Ferrand. Analyse de la concordance entre les données d'un questionnaire rempli par le médecin traitant avec celle de l'évaluation gériatrique multidimensionnelle faite par le médecin gériatre formé. 102 questionnaires ont été recueillis pour 200 patients. Une surcharge de travail est ressentie pour 53% des médecins. Le questionnaire est jugé pertinent danns 67% des cas où il est rempli. Sauf pour l'appréciation de la sévérité des troubles cognitifs (parfois perçus moins sévèrement par le médecin généraliste), les informations recueillies sur l'autonomie, les antécédents et le mode de vie lorsqu'elles sont renseignées sont comparables à l'évaluation faite par le gériatre. Lorsque les médecins généralistes sont guidés dans la réalisation d'une évaluation gériatrique multidimensionnelle, il existe une bonne capacité d'analyse des patients quelle que soit la sévérité des troubles. On peut donc supposer que l'élaboration d'une trame regroupant les différents items de l'évaluation gériatrique multidimensionnelle leur permettrait de mesurer tout ce qu'ils ont repéré et d'être systématiques sur les autres fragilités de leurs patients. Ainsi, un diagnostic plus précoce des troubles cognitifs en soins primaires serait facilité et de la même manière, le suivi des patients déments serait amélioré.

Introduction : avec le vieillissement démographique, les personnes de plus de 80 ans représentent le groupe d'âge progressant le plus rapidement et représentera d'ici 30 ans 1/5ème de la population mondiale. Il en résulte une augmentation de la demande de soins et aussi de passages aux urgences pour cette tranche d'âge, nécessitant de prises en charges spécifiques. Nous avons ainsi élaboré une évaluation gériatrique, utilisable aux urgences, basée sur des scores simples et validés, pour faciliter la prise en charge adaptée des patients gériatriques : orientation, transmission commune de l'information, mise en place de plan de soins, souhaits du patient. Matériels et Méthode : étude bi-centrique (Centre Hospitalier Universitaire (CHU) de Nice et Centre Hospitalier (CH) de Cannes), prospective, incluant les patients de 80 ans et plus, passant dans les services d'urgence, durant la période du 1 juin au 31 octobre 2020. L'objectif est d'évaluer la faisabilité d'une évaluation gériatrique dans les services d'urgences. L'évaluation comporte plusieurs scores et items dont le Clinical frailty scale (CSF), le score American society of anesthesiologists (ASA), le score de Glasgow, l'évaluation nutritionnelle, la présence de directives anticipées, l'accord pour une prise en charge en soins critiques, le lieu de vie et la présence d'aidants. Résultats : 161 évaluations ont été réalisées, avec un taux de remplissage de 1,49% sur le CHU de Nice et 3,09 % sur le Centre Hospitalier (CH) de Cannes dans la période considérée. La majorité des évaluations ont été réalisées en début de période. L'âge moyen des personnes inclus était de 88 ans. Nous retrouvons principalement des femmes vivant à domicile avec : un aidant, la présence de 6 antécédents et 5 traitements au long cours, une corpulence normale, un état de conscience conservé, un Clinical frailty scale moyen est de 4,4, un score American society of anesthesiologists de 2,5. Ces mêmes patients ont donné leur accord pour une prise en charge en soins critiques mais ne présentaient pas de directives anticipées. Conclusion : même si l'évaluation gériatrique aux urgences parait maintenant incontournable, notre étude a montré que sa réalisation, dans les conditions actuelles, peut encore être optimisée. Les prises en charge des patients âgés aux urgences sont souvent complexes et pluridisciplinaires, mais chronophages. L'intégration d'une expertise gériatrique et la création de filières de soins nous paraissent indispensables pour pouvoir appréhender au mieux cette population âgée encore grandissante dans les années à venir.
