Contexte : les avancées de la médecine ont fait reculer l'âge de la mort et les questions de la gestion de la fin de vie se posent plus que jamais. Entre partisans d'un accompagnement au chevet du malade et mouvements pro-euthanasie, la question divise. Comment expliquer que malgré une législation en place et une médecine contemporaine, le recours hospitalier reste l'issue majoritaire pour patients et familles. L'objectif est donc de s'interroger sur les facteurs limitant le décès à domicile des patients en fin de vie ayant exprimé un tel souhait. Quels sont les nouveaux rôles du médecin traitant pour aider ces patients dans leur prise de décision ? Méthodes : une étude qualitative avec entretiens semi-dirigés a été menée auprès de familles et soignants ayant été confrontés aux soins palliatifs au domicile. Le recrutement s'est fait à travers le Réseau de soins palliatifs RACYNES dans les Yvelines. Retranscription et analyse par axes thématiques saillants ont été réalisés. Résultats : 22 entretiens ont été menés. Malgré un souhait de maintien au domicile majoritaire (93.75%), le recours aux urgences est fréquent (45.5%) devant un manque de disponibilité du médecin traitant et une perte de confiance (31.25%). Les six soignants interrogés déplorent une formation insuffisante en matière de gestion de fin de vie et de soins palliatifs. Conclusion : les sociétés occidentales ont cherché à maîtriser la mort. Le bien mourir est aujourd'hui l'apanage de la cité, du politique et du législateur. Le médecin généraliste devient un médiateur, au centre de la prise en charge. Il apparait nécessaire de diffuser la culture palliative tout en tenant compte des enjeux de l'exercice libéral.
