Introduction : L'activité en oncologie médicale est en constante augmentation ces dernières années, avec une réelle croissance des hospitalisations non programmées alors qu'en parallèle le nombre de lits est resté stable. L'objectif principal de notre étude était de déterminer le taux d'hospitalisations non programmées en secteur de médecine. Le taux de réhospitalisations à 30 jours, d'hospitalisations évitables et les facteurs associés étaient des objectifs secondaires. Matériel et Méthodes : Une étude descriptive monocentrique a été menée dans un centre de lutte contre le cancer entre février et juin 2017. Tous les patients de plus de 18 ans hospitalisés de manière non programmée étaient inclus. Résultats : 291 hospitalisations non programmées ont été recensées lors de notre étude, soit 14,1% des hospitalisations totales sur la même période. Trois quarts des patients présentaient une maladie à un stade métastatique et 60% étaient traités par chimiothérapie. Le risque de réhospitalisation à 30 jours était estimé à 20,2%. La prise d'une immunothérapie était le seul facteur significativement associé à une réhospitalisation à 30 jours avec 4 fois moins de risque de réhospitalisation chez les patients traités. Le taux d'hospitalisations évitables était de 17,9%, sans mise en évidence de facteur significativement associé. Conclusion : Baisser le nombre d'hospitalisations non programmées et évitables en oncologie médicale suppose une réflexion sur la prise en charge globale du patient avec notamment le renforcement du lien ville-hôpital ou la mise en place de postes de bed manager dans les établissements.
