L’adolescence est marquée par la transition d’une dépendance infantile aux parents vers une autonomie et une maturité psychosociale nécessaires à l’accomplissement du rôle d’adulte. La complexité croissante de notre société crée des pressions psychologiques et socioculturelles sur les adolescents, favorisant une prise de risque physiologique, source d’opportunités et de vulnérabilité. Cette problématique de dépendance et de prise de risque est corrélée à une forte prévalence des conduites addictives à l’adolescence, source du paradoxe de l’adolescence, véritable problème de santé populationnel. La place du médecin généraliste, médecin référent des adolescents, est centrale dans le dépistage des addictions. Pourtant il existe une discordance entre données épidémiologiques et résultats de consultation de Médecine Générale. Cette discordance est expliquée en partie par l’absence de recommandation pour la pratique clinique du dépistage du risque addictif de l’adolescent, malgré la volonté gouvernementale d’action en amont de l’addiction. Par une revue compréhensive de la littérature réalisée via la base de données Medline et l’indexation MeSH sur la période 2000-2010, ce travail a permis de mettre en évidence 72 facteurs de risque biopsychosociaux potentiels de conduite addictive chez l’adolescent. L’interaction entre chaque facteur et objet d’addiction est étudiée. Sont discutés l’utilisation d’une analyse compréhensive de la littérature, ainsi que les résultats de l’étude en fonction de leur pertinence dans la pratique clinique et de leurs limites
