L'étude S.AGES, cohorte multicentrique prospective réalisée en vie réelle, avait pour but de décrire la prise en charge médicamenteuse en soins primaires de patients non institutionnalisés de plus de 65 ans. Notre étude se basait sur les données de l'étude S.AGES, et plus particulièrement sur la sous cohorte atteinte de fibrillation atriale. Elle avait pour but de décrire les facteurs associés à une hospitalisation. Les médecins recruteurs, qui étaient représentatifs des médecins généralistes en France, devaient effectuer avec leurs patients un suivi semestriel et rapporter les données à chaque visite. Sur 1 072 patients suivis, 43,28% ont été hospitalisés. Notre étude a permis de mettre en évidence un risque majoré d'hospitalisation en cas d'âge 80 ans, de polymédication 5 médicaments par jour, ou bien de maladie artérielle (critère composite avec notamment antécédent d'angor ou d'AVC). La sévérité d'une insuffisance cardiaque, la présence d'une pathologie de l'appareil locomoteur ou d'une hypertension artérielle n'entrainaient pas un surrisque d'hospitalisation. Ces résultats, en accord avec la littérature, soulignent l'importance du suivi des patients atteints de fibrillation atriale avec une coordination entre le spécialiste et la médecine générale de ville, ainsi que la nécessité de la lutte contre la polymédication

La Fibrillation atriale (FA) est le trouble du rythme cardiaque le plus fréquent, il touche 1 à 2% de la population générale et augmente avec l’âge. Selon les registres et études, 15% des personnes âgées de plus de 75 ans sont en FA. Les complications hémodynamiques et surtout thrombo-emboliques en font la gravité. Après rappel de la prise en charge diagnostique et thérapeutique des particularités du sujet âgé en FA et de la prise en charge actuelle recommandée du risque thrombotique et hémorragique, nous présentons l’étude prospective observationnelle d’une cohorte de patients hospitalisés en cardiologie conventionnelle au CHU de Reims. Le but de notre étude était de décrire mais aussi d’évaluer la prise en charge hospitalière du patient en FA de plus de 75 ans, en se focalisant essentiellement sur la stratégie de prise en charge du risque thrombo-embolique. Sur 1050 patients admis en cardiologie entre le 21 janvier et 21 mai 2010, 357 patients de plus de 75 ans ont été recensés ; 150 d’entre eux répondant aux critères d’inclusion (FA comme antécédent, motif d’admission ou élément émaillant l’hospitalisation) ont été inclus prospectivement (groupe FA) et leurs données comparées à celles des 207 autres patients (groupe sinusal). Les données cliniques, biologiques et échographiques conventionnelles étaient notées, ainsi que l’évaluation des scores de risque hémorragiques, thrombo-emboliques et des scores d’autonomie. Dans le groupe FA, on retrouve une augmentation significative des antécédents suivants : dysthyroidie, exogénose, maladies veineuses thrombo-emboliques et AVC. Dans le groupe FA 52,2% des patients étaient anticoagulés alors que la totalité d’entre eux avaient un score de CHA2DS2VASc ≥ 2 et 86% avaient un score de CHADS2 ≥ 2. Les scores de risque thrombo-emboliques n’étaient pas associés à la prescription d’anticoagulation alors que le score hémorragique HASBLED y était inversement associé. D’autres facteurs étaient associés significativement à la non-prescription de l’anticaogulation : le grand âge, le sexe féminin, l’augmentation de la créatininémie, la perte d’autonomie, la démence, le risque de chute. Une évaluation du risque thrombo-embolique et hémorragique a été réalisée à l’aide des scores de risque montrant que les patients étaient sous-traités : plus de 50 patients n’étaient pas anticoagulés alors que le risque thrombo-embolique prédit était supérieur au risque hémorragique, seuls 5 patients étaient anticoagulés alors que le risque hémorragique semblait supérieur. La morbi-mortalité cardio-vasculaire et particulièrement les évènements hémorragiques et thrombo-emboliques ont été évalués à 6 mois pour les patients du groupe FA : on recense 1 accident thrombo-embolique non fatal et 2 accidents hémorragique fatals. La personne âgée en FA est peu étudiée, la prescription de l’anticoagulation est souvent un choix difficile malgré les recommandations, de nombreux éléments propres à ces patients faisant modifier l’attitude thérapeutique. Le risque hémorragique est peut être surestimé par le prescripteur chez ces patients fragiles même si les événements tant thrombo-emboliques qu’hémorragiques recensés étaient peu nombreux dans notre étude.
