Introduction : de nombreuses études ont révélé l'impact négatif de l'AMP sur la vie sexuelle des patients. De nombreux patients sont concernés par cette altération de leur qualité de vie. Il apparait nécessaire d'interroger les patients et de les orienter pour leur apporter une prise en charge de leur santé sexuelle et reproductive. Matériel et méthodes : au cours de l'étude (quantitative, prospective, multicentrique, de décembre 2018 à février 2019), nous avons contacté tous les secrétariats des centres d'AMP cliniques recensés par l'ABM pour que ces derniers diffusent le questionnaire à leurs différents médecins par mail. Le questionnaire était disponible sur un site internet dédié et il a également été diffusé sur la page d'accueil du site de la Société de Médecine de la Reproduction. Résultats : nous avons recueilli 171 réponses au questionnaire qui comportait 16 items. Les médecins ayant répondu étaient pour 89 % des femmes, l'âge médian était de 34 ans [28-64]. 56% abordent toujours la sexualité au cours de leur première consultation, 30 % souvent, 14 % parfois. Nous avons établi deux groupes d'étude. Le groupe Abord Régulier (AR) qui comprend les médecins (n= 147) ayant répondu « oui, toujours » et « souvent ». Et le groupe Abord Occasionnel (AO) pour ceux (n=24) ayant répondu « parfois ». Le groupe AR aborde moins souvent la programmation des rapports sexuels que le groupe AO (72 % versus 91 % respectivement, p=0,029). On trouve des différences significatives entre les deux groupes AR et AO pour la recherche de la fréquence des rapports sexuels (99 % versus 83 %), les troubles de la pénétration (53 % vs 29 %) et les troubles du désir (42 % vs 21 %). 37 % et 21 % ont un sexologue dans leur centre, 64 % et 67 % adressent les patients en libéral. Et 9 % et 17 % ne savent pas à qui ni où les adresser. 93 % et 75 % des groupes AR et AO pensent qu'un sexologue en AMP serait utile dans leur centre. Conclusion : cette étude met en exergue le manque d'intérêt et de moyens humains pour la prise en charge sexologique des patients. Nos résultats peuvent inciter les médecins de la reproduction à améliorer la prise en charge de la santé sexuelle et reproductive des patients, pour permettre aux couples conjugaux de s'épanouir au mieux en couples parentaux.

Surrogacy is India's new form of outsourcing, as couples from all over the world hire Indian women to bear their children for a fraction of the cost of surrogacy elsewhere with little to no government oversight or regulation. In the first detailed ethnography of India's surrogacy industry, Amrita Pande visits clinics and hostels and speaks with surrogates and their families, clients, doctors, brokers, and hostel matrons in order to shed light on this burgeoning business and the experiences of the laborers within it. From recruitment to training to delivery, Pande's research focuses on how reproduction meets production in surrogacy and how this reflects characteristics of India's larger labor system. Pande's interviews prove surrogates are more than victims of disciplinary power, and she examines the strategies they deploy to retain control over their bodies and reproductive futures. While some women are coerced into the business by their families, others negotiate with clients and their clinics to gain access to technologies and networks otherwise closed to them. As surrogates, the women Pande meets get to know and make the most of advanced medical discoveries. They traverse borders and straddle relationships that test the boundaries of race, class, religion, and nationality. Those who focus on the inherent inequalities of India's surrogacy industry believe the practice should be either banned or strictly regulated. The aim of this book is twofold: to promote an awareness of cultural differences in connection with sexual medicine among health care providers, and to demonstrate how such differences are relevant to the care and treatment of patients with sexual issues. Individual sexuality represents the cumulative effects of biological, psychological, and cultural influences. Biological immunity as we know it does not exist until the late nineteenth century. Nor does the premise that organisms defend themselves at the cellular or molecular levels. For nearly two thousand years “immunity,” a legal concept invented in ancient Rome, serves almost exclusively political and juridical ends. “Self-defense” also originates in a juridico-political context; it emerges in the mid-seventeenth century, during the English Civil War, when Thomas Hobbes defines it as the first “natural right.” In the 1880s and 1890s, biomedicine fuses these two political precepts into one, creating a new vital function, “immunity-as-defense.” In A Body Worth Defending, Ed Cohen reveals the unacknowledged political, economic, and philosophical assumptions about the human body that biomedicine incorporates when it recruits immunity to safeguard the vulnerable living organism. Inspired by Michel Foucault’s writings about biopolitics and biopower, Cohen traces the migration of immunity from politics and law into the domains of medicine and science. Offering a genealogy of the concept, he illuminates a complex of thinking about modern bodies that percolates through European political, legal, philosophical, economic, governmental, scientific, and medical discourses from the mid-seventeenth century through the twentieth. He shows that by the late nineteenth century, “the body” literally incarnates modern notions of personhood. In this lively cultural rumination, Cohen argues that by embracing the idea of immunity-as-defense so exclusively, biomedicine naturalizes the individual as the privileged focus for identifying and treating illness, thereby devaluing or obscuring approaches to healing situated within communities or collectives.
