Introduction : Actuellement, seuls 26 % des décès ont lieu à domicile alors que l’une des dernières volontés de ces patients est de mourir chez eux. Le médecin généraliste, parfois démuni face au patient et à sa famille, peut avoir recours aux réseaux de soins palliatifs pour l’aider au mieux dans sa prise en charge. L’objectif de ce travail est de déterminer les raisons d’un recours souvent tardif au réseau de soins palliatifs en médecine générale mais aussi les outils à apporter pour favoriser un recours plus précoce. Matériel et méthodes : Etude qualitative réalisée sous forme d’entretiens semi-directifs auprès de médecins généralistes de la Somme ayant une convention ou non avec le réseau PALPI 80. Entretiens anonymes, effectués au cabinet des médecins généralistes après tirage au sort en octobre 2015. Résultats : La majorité des praticiens assimile encore soins palliatifs avec fin de vie. Ils éprouvent des difficultés dans ces prises en charge complexes, chronophages, les impliquant tout autant professionnellement qu’émotionnellement. Ils rapportent également un manque de coordination ville-hôpital ainsi qu’un manque de communication entre les différents intervenants au domicile. Discussion : Il est nécessaire pour les médecins généralistes d’améliorer la communication ainsi que l’organisation ville-hôpital et de pouvoir proposer des formations pour diffuser cette culture palliative, leur permettant d’anticiper la prise en charge commune. Conclusion : La pluridisciplinarité des réseaux de soins palliatifs est un atout majeur pour le médecin traitant dans la prise en charge de ses patients au domicile.
