Cette réforme passe par la concentration des spécialités opérationnelles sur les plateformes hospitalières Ile de France et Provence-Alpes-Côte d'Azur et par une réduction du format des hôpitaix d'instruction des Armées périphériques devenus établissements civils et militaires hospitaliers. L'objectif est de mesurer l'impact de la transformation hospitalière sur le parcours de soins du militaire coordonné par le médecin généraliste militaire. Nous avons repris dans notre étude descriptive et comparative le questionnaire de l'état des lieux du parcours de soins du militaire entre CMA et HIA par Emeric Romary en 2012, et l'avons distribué aux médecins responsables des Centres médicaux des Armées et des Antennes médicales. Nous avons recueilli un taux de réponse de 25% (60/232) entre avril et juin 2018. L'étude ne montre pas de différences significatives de recours à l'HIA entre 2012 et 2018 mais objective une augmentation des délais de rendez-vous et des remises de compte-rendu malgré la mise en place de filière dédiée aux militaires. 25% des médecins plébiscitent la rapidité de prise en charge en HIA en 2018 contre 83% en 2012, 31% évoquaient l'existence d'une structure d'accueil en 2018 contre 12% en 2012, 8% pour la rapidité des compte-rendu en 2018 contre 44% en 2012. Les pistes d'amélioration sont la poursuite de la création de filière dédiée aux militaires, la modernisation des moyens de prise de rendez-vous et d'échange avec le spécialiste avec de nouvelles technologies de l'information et de la communication.
