Introduction : Afin de limiter le nombre de passages aux urgences de patients relevant d'une consultation de médecine générale, un protocole de réorientation a été mis en place. Sur les horaires de permanence de soins, le patient est invité, après une évaluation par l'infirmière organisatrice de l'accueil des urgences, à composer le 116 117. Ce protocole est sujet à controverse notamment concernant la satisfaction de ces patients. L'objectif principal de l'étude était d'évaluer la satisfaction globale des patients réorientés. Méthode : Une enquête de satisfaction téléphonique observationnelle, prospective, monocentrique sur l'hôpital de La Roche-sur-Yon a été réalisée sur 105 jours avec un appel des patients 3 à 5 jours après la réorientation. La satisfaction globale des patients était évaluée, ainsi que leur satisfaction dans les différents aspects des soins. Le ressenti des patients vis-à-vis de la justification de cette réorientation mais également la compréhension de cette dernière étaient étudiés. L'effet néfaste qu'avait pu avoir cette réorientation sur leur santé, tout comme leur parcours de soins futur en cas de problématique relevant de la médecine générale étaient aussi recherchés. Résultats : 127 patients ont été inclus dans l'étude qui s'est déroulée du 18 juin au 30 septembre 2019. 93 patients ont répondu au questionnaire de satisfaction. Parmi eux, 90 % ont appelé le 116 117 et ont été régulés : 33 % ont bénéficié d'un conseil médical et 67 % ont été orientés vers un médecin de garde. 77 % des patients se sont estimés globalement satisfaits de leur prise en charge. Les patients ayant reçu un conseil médical étaient 59 % à être globalement satisfaits contre 91 % chez les patients vus par un médecin généraliste. 81 % des patients ont déclaré avoir compris la réorientation et 76 % l'ont estimée justifiée. Concernant le processus de régulation, 60 % des patients ont estimé la durée d'attente téléphonique comme bonne ou excellente. 13% des patients ont estimés que leur réorientation avait eu un effet néfaste sur leur santé, parmi eux 5 % ont déclaré avoir eu une aggravation de leur pathologie mais aucun n'a nécessité d'hospitalisation. Dans le futur, 47 % rappelleront le 116 117 en cas de problématique relevant de la médecine générale. Conclusion : Les patients se sont estimés globalement satisfaits de leur prise en charge et ce d'autant plus lorsqu'ils ont pu être vus en consultation par un médecin de garde. La réorientation semble comprise, acceptée par les patients et plutôt sécurisée. Le protocole pourra être poursuivi. L'accent devra être mis sur l'information des professionnels de santé vis-à-vis de ce protocole, mais également sur l'information et l'éducation des patients au sujet du 116 117 et de l'organisation de la permanence des soins ambulatoires.
