Les hémorragies sous anticoagulant oraux (ACO) sont des motifs fréquents de consultation aux urgences et leur prise en charge est codifiées par des recommandations ou propositions d'experts. En cas d'hémorragie grave sous ACO et en l'absence d'antidote, les agents utilisés pour antagoniser l'effet anticoagulant sont des facteurs procoagulants : complexes prothrombiniques activés (CCPa) ou des concentrés de complexes prothrombiniques (CCP). Comparaison des paramètres de génération de thrombine (GT) chez 3 populations de patients sous ACO : patients avec accidents hémoragiques graves ayant reçu des facteurs pro-coagulants dans le cadre de la réversion, patients avec hémorragie non grave sans réversion et patients anticoagulés sans accident hémorragique. Etude prospective, descriptive, monocentrique de la GT chez les patients présentant une hémorragie non grave (non reversée) ou mettant en jeu le pronostic vital et/ou fonctionnel sous ACO (AVK, rivaroxaban, dabigatran, apixaban) réversée par CCP, CCPa ou idarucizumab. L'étude a été réalisée sur plasma pauvre en plaquettes prélevé à l'arrivée (V1) avant la réversion, puis 30 min (V2), 6h (V3) et 24h (V4) après la réversion. 307 patients ont été inclus dans cette étude (98 patients avec hémorragie grave réversés dont 61 AVK, 13 rivaroxaban, 17 apixaban et 7 dabigatran), 95 patients sans hémorragie grave non réversés (44 AVK, 25 rivaroxaban, 24 apixaban et 2 dabigatran), 108 patients contrôles sous ACO sans saignement). Pour les V1, les patients sous AVK avec hémorragie réversée présentaient une diminution significative des principaux paramètres de GT (ETC et pic) en comparaison aux patients sans hémorragie (p =0, 017). Il ne semblait pas exister de différence significative pour le rivaroxaban l'apixaban et le dabigatran entre les 3 populations à V1.
