Ce mémoire a pour but d’analyser l’évolution longitudinale des performances dans des épreuves d’accès lexical chez des sujets atteints de la maladie d’Alzheimer (MA) bénéficiant de différentes prises en charge. Ainsi, un protocole constitué d’épreuves de fluence verbale et de tests de dénomination d’images (DO 80, BIMM, planche d’évocation créée) a été proposé à 38 patients MA âgés de 70 à 90 ans. Trois groupes de MA se distinguent par leur suivi : stimulations en groupe du langage, autres stimulations, sans stimulation. Nous avons comparé leurs résultats à un groupe à un stade plus sévère de la démence ainsi qu’à 10 sujets témoins du même âge. L’analyse des résultats montre une diminution significative des performances au niveau des scores et des temps de réponse et met en évidence les bénéfices de la stimulation orthophonique sur l’accès lexical. Nous soulignons au niveau des variables observées, un effet de la fréquence du mot et de la couleur de l’image sur la dénomination au détriment de l’absence d’effet de la longueur du mot et de la classe grammaticale.

Un protocole constitué d' un test de dénomination d' images (BIMM) et d' un test non validé d' évocation de synonymes, d' antonymes et d' évocation sur définition (Télexab) a été proposé à 14 sujets témoins âgés de 71 à 88 ans ainsi qu' à 14 patients du même âge. Nous avons comparé leurs résultats. L' analyse des résultats a tout d' abord révélé que Télexab met en évidence le manque du mot au même titre que la BIMM, mais nous avons retrouvé des différences dans la nature des erreurs et des réponses produites en fonction de la modalité auditive (Télexab) ou visuelle (BIMM). De plus, cette étude a permis de montrer que Télexab met en évidence les difficultés des sujets en situation quotidienne ainsi que leurs stratégies pour tenter de pallier à leur manque du mot. Nous soulignons également un effet de classe grammaticale dans les deux épreuves ainsi que la mise en évidence d' une meilleure préservation des aspects sémantiques en début d' évolution.
