To observe the success rates of the implants and esthetic outcomes after finally restoration.Materials and MethodsA total of 5 patients (7 sites) were included with Type II sockets in the anterior maxillary. Flapless immediate implantation and GBR were performed in the sockets at the same time. The implant-supported temporary crown or a custom-healing abutment was attached at the day of surgery. 6 months later, taking impression and finishing the final restoration. Followed by a period of 10 months to 45 months after final restoration. The success rate and red-white esthetic outcomes were observed. X-ray examinations were performed to observe changes in the marginal bone level around the implant.ResultatsNone of the 7 implants loosen or falled off during the follow-up period. The implant function was normal and the implant success rate was 100%. After the permanent restoration, good aesthetic effects were obtained. The natural contour and the mesial-distal gingival papilla were completely preserved. The highest point of the labial margin was in harmony with the height of the adjacent natural teeth, and the alveolar process was full and natural. After 10 months to 45 months of follow-up, the stability of the implants was good. The X-ray showed that marginal bone level around the implants was stable.Conclusions and Clinical ImplicationsFor the Type II sockets, flapless immediate implantation and GBR were performed in the sockets at the same time. Using an implant-supported temporary crown or a custom healing abutment to seal the extraction. After final restoration, a good implant success rate and red-white esthetic outcomes can be obtained.

Background:Immediate implant placement into fresh extraction socket especially in the esthetic area have reduced the treatment time, surgical procedure, and it has increased the comfort of the patient. This is achived thanks to improvement in implant designed and using a precisal surgical and prosthetic protocol. The Trabecular Metal Implant is made with a midsection of Tantalium based biomaterial, that is 80% porous, with a trabecular structure designed for bone ongrowth and bone ingrowth .Pourpose:This retrospectiv study was conducted to evaluate the clinical efficency and radiological measurements of immediate placement and occlusal loading of single-tooth restorations on hybrid titanium dental implants with highly porous, unthreaded, tantalum-based midsections in fresh extraction sockets, positioned from January to December 2013.Materials and Methods :A retrospective review of patient records was conducted in a single private-practice setting at 1,2 and 4 years. Single, non restorable teeth were atraumatically extracted using an incision-free, flapless tecnique. Reasons for extraction included crown/root fractures, deep caries and/or endodontic failures. Implants were lingually placed 3-4 mm apically to the free gengival margin in fresh extraction sockets and the gap around the implants were grafted. The length of the implants ranged from 10-13 mm and the implant diameter ranged from 4.1 to 6.0 mm, A temporary crown screwed in acrilyc resin was immediately delivered and placed into occlusion. Two weeks later, implants were definitively restored with glass ceramic crowns. Results:Twenty-six patients were treated for one non-restorable tooth (n=26). Most implant were placed in maxillary (n=16/26) and premolar (n=17/26) locations, and in lower density (Type 3) bone (n=23/26). All implant insertion torque levels were >40 Ncm (n=26/26). Mean ISQ values were 71.6 immediately after placement. All implants were provisionalized immediately with screw-retained crows (n=26/26). Pink Esthetic Scores were 13.4 at 1 year and 13.0 at the 4-year follow-up. Mean peri-implant probing were 2,35 mm+- 2.4 at 1 year and 2,92 mm +- 2.6 at 4 years of follow-up. Mean crestal bone loss was 0.58 mm +- 0.34 at 1 year and 0,869 mm +- 0.36 at the 4-year follw-up. Implant survival and success rates were 100% (n=26/26), respectively, with a follow-up time of 4 years. There were no irresolvable complications. All patients reported full satisfaction for mastication function, phonetics, and esthetics.Conclusion:With the limits of this retrospective study, the use of Tantalum-based Implants in post-extractive sites with immediate provisional restoration turns out to be a safe procedure and predictable over the long term with stability of hard and soft tissues.

Abstract: Background: Type I immediate implant placement has gained popularity because it may reduce treatment time, number of surgeries and post-extraction bone loss. However, this is potentially challenged by inadequate keratinized mucosa for flap adaptation and difficulties in achieving primary stability. Moreover, it has been proven that post-extraction bone loss is an inevitable biological process, which affects treatment outcomes. Objectives: To estimate survival and success rates of implants and the implantsupported prostheses, the prevalence of biological, technical and aesthetic complications, and the magnitude of soft and hard tissue changes following implant placement immediately into fresh extraction sockets. Material & methods: An electronic search in MEDLINE (PubMed) and the Cochrane library from 1991 to July 2010 was performed to include prospective studies on immediate implants with a mean follow-up time of at least 1 year. The survival rates were computed using the STATA statistical software. Weighted means of soft and hard tissue changes were obtained by the inverse variance method. Results: A total of 46 prospective studies, with a mean follow-up time of 2.08 years, were included. The annual failure rate of immediate implants was 0.82% (95% CI: 0.48%-1.39%), translating into the 2-year survival rate of 98.4% (97.3%-99.0%). Among the 5 factors analyzed [reasons for extraction, antibiotic use, position of implant (anterior versus posterior, maxilla versus mandible), type of loading], only the regimen of antibiotic use affected the survival rate significantly. Lower failure rates were found in groups that were provided with a course of post-operative antibiotics. The success of implant therapy was difficult to assess due to scarce reporting on biological, technical and aesthetic complications. Soft tissue changes occurred mostly in the first 3 months after the provision of restoration, and then stabilized towards end of the first year. Marginal bone loss predominantly took place in the first year after implant placement, with a magnitude generally less than 1mm. Controversy on hard tissue preservation with platform-switching technique remained unsolved. Conclusions: Despite the high survival rate observed, more long-term studies are necessary to determine the success of implant treatment provided immediately after tooth extraction. Bony defects due to chronic inflammation or trauma carry the potential to compromise the final esthetic result of implanttherapy.Aim/Hypothesis:The aim of the present comparative clinical trial was to evaluate immediate implants in the esthetic zone of the maxilla in the presenceor absence of buccal bone defects regarding esthetic outcome and bone remodeling.Materials and Methods:A total of 24 patients (mean age: 56 years) were subjected to immediate implant placement in the maxillary incisor or canine region.12 implants were placed into intact sockets and 12 implants were inserted next to bone defects (5 mm in apicocoronal and 4 mm inmesiodistal direction, on average). All implants were immediately restored using zirconia Copy-abutments and followed-up for 1 yearof function.Results:No difference in median Pink Esthetic Scores (PES) were observed between the defect and non-defect group (11.5 vs. 12.0,p=0.336). A higher rate of alveolar deficiency (PES-5, +29%) and soft tissue discoloration (PES-7, +17%) in implants with bonydefects and a weak correlation betweeen PES and defect size (r=0.71, p=0.018) was seen. No difference regarding mucosalrecession (0.6 vs. 0.8 mm) as well as marginal bone loss (1.6 vs. 1.4 mm) could be observed.Conclusions and Clinical Implications:No differences regarding esthetic outcome and peri-implant bone remodeling could be substantiated between immediate implants insockets with vs. without buccal bone defects, however, implant esthetics may be less predictable in cases of bony deficiencies.
