Objectif : Les enfants présentant des fentes labiales et/ou palatines peuvent être porteurs de malformations associées entrant ou non dans un cadre syndromique. L'objectif de ce travail est d'étudier la distribution des malformations et syndromes associés aux fentes labiales et/ou palatines. Matériel et méthode : Il s'agit d'une étude rétrospective de 324 patients porteurs de fentes labiales et/ou palatines, nés entre 1994 et 2001. Résultats : Les fentes labiales ou labio-alvéolaires sont plus rarement associées à d'autres malformations que les fentes à atteinte palatine. Ces malformations associées touchent préférentiellement le système néphro-uro-génital pour les fentes palatines et le système cardio-vasculaire pour les fentes labiales. Les fentes labiales et labio-alvéolaires sont rarement syndromique (3,46%), à la différence des fentes palatines qui entrent dans un cadre syndromique dans plus de 34% des cas. Conclusions : La prévalence des malformations semble liée d'une part au type de fente, l'atteinte du palais est associée à un taux plus élevé de malformation associée, et d'autre part à la présence d'un syndrome. Ces constatations soulignent l'intérêt de la réalisation d'un bilan malformatif général dans les cas de fentes a fortiori à atteinte postérieure, afin de diagnostiquer les malformations et de pouvoir organiser une prise en charge efficace et cohérente.

At first sight it may appear strange that a volume of Progress in Pediatric Surgery should be devoted to the history of our specialty. One assumes that progress is concerned primarily with recent developments whilst history deals with matters of the past. However, in the past there has also been considerable progress in the development of our understand ing of paediatric surgical problems, otherwise we would not have pro gressed to our present achievements. The editors, therefore, do not apologize for compiling this volume but, on the contrary, feel that the publication of this volume is most timely. Modern paediatric surgery has now been practised for three genera tions. The handful of pioneers who were the founders of our specialty worked mainly before the last world war. A few dozen of the inter mediary generation started work immediately after the war, while the new generation who are now dominating our specialty must be counted in thousands. Two factors have radically altered paediatric surgery as practised by the intermediary and the present generation of surgeons.

For more than a century, the condition now known as Idiopathic Hydronephrosis has been recognised as a clinical entity, and following the original description by Rayer in 1841 a variety of procedures were devised in attempts to correct the condition surgically. Most of these early methods were introduced in the last decade of the nineteenth century by several illustrious clinicians, including Trendelenburg, KOster, Fenger and Sutton. For many years diagnosis was based purely upon the patients presenting signs and symptoms and not until the early part of this century was technology available to assist in the pre-operative diagnosis of the condition. Early methods depended upon radiological techniques, and the introduction of the retrograde pyelogram by Voelcker and Lichtenberg in 1906 represented a significant advance in diagnostic methodology. Other methods also dependent upon radiographic techniques were subsequently introduced, including urography in the late 1930s by Swick, and more recently, the method of cineradio graphy, as pioneered with considerable success by Peter Narath in the decade following World War II. During the past 50 years a variety of surgical procedures have been introduced for the treatment of idiopathic hydronephrosis. That so many different methods have been devised suggests that no one specific technique is capable of achieving a complete cure in all cases."
