La myomectomie est un des principaux traitements des fibromes utérins symptomatiques. Elle peut-être responsable d'une importante spoliation sanguine lorsqu'elle concerne des fibromes multiples, volumineux ou interstitiels. L'embolisation pré-opératoire des fibromes utérins, technique de radiologie interventionnelle ayant prouvée son efficacité dans les hémorragies de la délivrance et du post-partum, diminue significativement les saignements per et post-opératoires. En 1995, deux premières séries d'embolisation pré-opératoire des fibromes utérins ont été publiées. Ces deux enquêtes de faisabilité ont permis de mieux connaître la vascularisation et les variations anatomiques des myomes utérins, les causes des chirurgies hémorragiques et l'innocuité d'un cathétérisme artériel sélectif sur l'utérus et ses annexes. Cette nouvelle série multicentrique de 47 nouveaux cas, confirme que l'embolisation pré-opératoire permet une chirurgie quasiment exsangue, même pour les fibromes de grandes tailles : la spoliation sanguine a été réduite dans 87 % des cas. De plus, cette technique permet une grossesse ultérieure (7 grossesses rapportées). D'autre part cette nouvelle série a permis de mieux cerner les indications et les limites de cette préparation à la chirurgie : elle ne doit rester indiquée que pour des patientes présentant des fibromes à haut risque hémorragique. C'est une technique, qui bien qu'efficace, nécessite un plateau technique lourd et spécialisé, et les particules utilisées doivent être rapidement résorbables. Il reste donc à savoir, à l'aide d'une étude prospective, si elle garde une place particulière parmi les différentes méthodes de préparation à la chirurgie des fibromes utérins à haut risque hémorragique, notamment par rapport aux analogues.

OBJECTIF : Analyser à long terme la réduction de volume et la dévascularisation des fibromes utérins traités par embolisation des artères utérines, ainsi que l’amélioration clinique. MATERIELS ET METHODES : Etude rétrospective portant sur l’analyse d’une série de 26 patientes consécutives porteuses d’un ou plusieurs fibromes utérin symptomatiques traités par embolisation des artères utérines. Les 26 patientes ont été prises en charge dans le service de radiologie de l’hopital Maison Blanche. Les analyses du volume et de la vascularisation des fibromes ont été effectuées sur les IRM réalisées avant le geste puis sur les IRM de contrôle à 6, 18, 30 puis 42 mois. Nous avons également recueilli les données cliniques rapportées par un interrogatoire réalisé trois mois après le geste. RESULTATS : La procédure s’est déroulée avec succès chez 25 patientes. 92% des patientes ont rapporté une amélioration satisfaisante des symptômes. L’efficacité était de 88% pour les saignements et de 88% pour le syndrome de masse pelvien. La dévascularisation du fibrome a été complète dans 88% des cas, partielle dans 8% et nulle dans 4% (1 cas). La réduction du volume des fibromes dominants était en moyenne de 49% entre les deux premières IRM et de 31% entre la deuxième et la troisième. Les différences sont apparues moins importantes et peu significative lors des contrôles suivants. Il n’a pas été en évidence de facteur pouvant avoir une influence significative sur ces résultats (taille, localisation et signal des fibromes lors de la première IRM). De même, l’association avec une adénomyose ne semble pas avoir été un facteur de risque de moindre efficacité. CONCLUSION : L’embolisation des artères utérines est donc une alternative satisfaisante à la chirurgie. Cette méthode apparaît efficace et sure. Le suivi en IRM a permis d’évaluer le comportement à long terme des fibromes embolisés. Ainsi l’essentiel de la réduction de volume est observée pendant les 18 premiers mois. La variation est ensuite beaucoup plus faible et moins significative.

In the ancestral environment, a human female typically carried at least half a dozen babies to term. The fact that modern women are able to limit the number of children they bear has dramatic consequences for the incidence of uterine fibroids, as well as the clinical care of fibroids patients. Fibroids, Menstruation, Childbirth, and Evolution explores these connections, integrating a vast amount of medical knowledge about the uterus into one volume. During pregnancy, the mother's blood prepares for an enormous hemostatic event: the delivery of the placenta. That fetal organ is the vascular link between mother and offspring. At childbirth, one-tenth of mother's cardiac output flows through the placenta, feeding the growing child. When the placenta is sheared away from its attachment to the uterus, two hundred large uteroplacental arteries are ripped apart and bleed profusely into the uterine cavity. For many hours following delivery, uterine contractions slow blood flow within the uterus, allowing the high concentration of clotting factors built up in the mother's blood during pregnancy to solidify throughout the uterine circulation and stop blood loss. Then, hours later, the tide reverses, most of these uterine blood clots dissolve, and more normal blood flow returns to the uterus. This amazing process occurs with each pregnancy. During this process, the uterus is ischemic and hypoxic. Unlike brain and heart, which can only survive minutes of decreased blood flow, the uterus can withstand dramatically diminished blood flow for hours. In fact, it is natural for this to occur once every few years. Uterine ischemia and hypoxia are a natural part of every woman's genetic makeup. In 1995 a group of French physicians discovered that it was possible to emulate the physiology of childbirth by stopping blood flow to the uterus with small plastic particles. Initially, they injected these particles to diminish blood loss during subsequent fibroid surgery. However, they soon learned that the injection of these particles was therapeutic in and of itself for women with symptomatic fibroids. Unbeknownst to this French group, earlier, in 1964, an American physician surgically occluded the uterine arteries to treat women without fibroids who had excessive monthly menstrual blood loss. Subsequent physicians have occluded the uterine arteries in various ways to treat a third common disorder, adenomyosis. Finally, these clinical successes suggest that future episodes of endometriosis may be preventable in some women treated with uterine artery closure.
