Contexte : La Communication Médecin Malade (CMM) est une composante essentielle de l'efficacité des soins. Elle influe sur la satisfaction et l'adhérence du patient. Il n'a pas été retrouvé dans la littérature d'outils génériques faisant consensus pour en mesurer la qualité. Objectifs : Créer une échelle évaluant la CMM au cours d'une consultation et décrire ses déterminants. Méthodes : Une recherche bibliographique a permis de retenir 20 échelles validées, à partir desquelles ont été sélectionnés par une équipe multidisciplinaire les items pertinents, permettant d'aboutir à un questionnaire à 15 items : échelle CMM-15. La validation des propriétés intrinsèques et la recherche des déterminants de la CMM ont été réalisées par une étude sur 150 patients qui consultaient dans deux services d'urgences pour des pathologies aiguës. Résultats : La cohérence interne de l'échelle est élevée (alpha de Cronbach à 0.89). Les déterminants de la CMM sont le niveau d'information (conseils hygiéno-diététiques ; critères de reconsultation) (p = 0.01) et l'appartenance du patient au milieu médical (p = 0.04). Conclusion : Cet outil générique pourrait permettre d'évaluer l'impact d'interventions au cours d'une consultation et être utilisé comme un indicateur de la qualité des soins. Pour valider les propriétés extrinsèques, l'échelle devra être testée dans d'autres contextes cliniques. L'information est le principal déterminant de la qualité de la communication et a un effet multiplicatif sur le score de CMM qui lui-même à un effet sur l'adhérence globale du patient. Un des axes d'amélioration de la communication réside dans la qualité de l'information délivrée aux cours d'une consultation.
