Afin 1) de déterminer un niveau d'activité optimal pour la structure et 2) d'évaluer les conséquences économiques des améliorations organisationnelles, notre objectif était de construire un modèle d'analyse des coûts du bloc opératoire (par exemple, impact financier des retards des débordements du programme opératoire, et estimation des gains espérés grâce à leur diminution), puis de situer nos résultats dans une analyse systématique de la littérature internationale sur l'efficience des blocs opératoires. Des obstacles méthodologiques, décrits dans cette thèse, nous ont conduits à résoudre notre problématique de manière indirecte. Nous avons d'abord mis en évidence un surcoût important tant par rapport à la référence de l'échelle nationale des coûts (ENC) que par rapport aux recettes, ce surcoût portant majoritairement sur les postes de personnel. Nous avons ensuite estimé la possibilité de générer des recettes supplémentaires en augmentant l'activité. A cette fin, quatre scénarii ont été proposés et leurs résultats chiffrés. A défaut de pouvoir élaborer un modèle économique intégré et généralisable, nous avons donc pu calculer, non les coûts, mais les recettes perdues à cause des défauts d'organisation. Notre revue systématique de la littérature replace ce travail dans son contexte et permet de discuter les difficultés rencontrées, la pertinence de nos résultats et les moyens de leur mise en œuvre.
