Introduction: L'incidence du diabète de type 1 est en augmentation chez l'enfant surtout chez les moins de 4 ans avec une exposition plus précoce aux complications liées au diabète. Patients et méthodes: L'objectif de notre travail est de décrire l'équilibre métabolique et les complications microvasculaires et macrovasculaires à l'âge adulte dans une cohorte de 352 patients diabétiques de type 1 diagnostiqués à l'âge pédiatrique. Il s'agit d'un suivi longitudinal prolongé, descriptif établi grâce au dossier médical informatisé CARéDIAB. Résultats: L'âge moyen des patients est de 31 ans, la durée d'évolution moyenne du diabète est de 21,1 ± 11,2 ans. L'HbA1c moyenne globale est de 8,7±1,32%. 89 patients (25%) ont une rétinopathie après 21 ans d'évolution, 34 patients (9,7%) ont eu de la photocoagulation rétinienne. L’impact de la période pédiatrique de la prise en charge actuelle du diabète sur la survenue de complications à l’âge adulte reste mal établi. Objectif : Définir l’impact de l’équilibre glycémique pédiatrique dans la survenue de complications à long terme chez des patients diabétiques de type 1. Patients et méthodes : Etude de cohorte prospective sur 195 patients suivis au CHU de Rennes et ayant une durée de diabète supérieure à 10 ans. Les complications recherchées étaient la rétinopathie, la néphropathie, la neuropathie et la macroangiopathie diabétique. Résultats : A une durée moyenne de diabète de 16.2 (+/- 4.5) ans, le pourcentage de complications était de 19.5%, 14.3% pour la rétinopathie diabétique et 7.2% pour la néphropathie diabétique. Aucun patient n’a présenté de macroangiopathie. En analyse multivariée, l’équilibre glycémique global apparaissait comme un facteur majeur de risque de complications (HR 3.8), et en analysant les données pédiatriques et adultes, seul l’équilibre pédiatrique apparaissait statistiquement significatif (HR 1.8).
