Introduction : La gastroentérite aigue à rotavirus est responsable de nombreuses consultations et hospitalisations chaque année en France. Il existe cependant deux vaccins efficaces mais qui ne sont ni recommandés ni remboursés en France. Différentes études ont montré que les médecins généralistes ne proposaient pas la vaccination. Nous avons voulu déterminer les freins à la réalisation de cette vaccination chez les médecins généralistes d'Ille-et-Vilaine. Matériels et méthodes : Nous avons réalisé une étude observationnelle descriptive transversale auprès des 1148 médecins généralistes libéraux installés ou remplaçants d'Ille-et-Vilaine. Un questionnaire a été mis en ligne sur le site Limesurvey® entre février et avril 2022 et a été diffusé par différents moyens. Résultats : Nous avons recueilli 173 questionnaires. 72,99% des médecins interrogés ne proposent pas la vaccination. Les deux freins principaux sont l'absence de remboursement (81,61%) et de recommandations (78,16%). Viennent ensuite le caractère non obligatoire (75,87%), le doute sur la balance bénéfice/risque (74,71%), les effets indésirables possibles (73,56%) et le manque d'informations (65,52%). Enfin la surcharge du calendrier vaccinal (48,85%), le faible impact de la gastroentérite aigue à rotavirus en terme de santé publique (44,83%) et le probable refus des parents (42,53%) apparaissent comme des freins secondaires. Nous remarquons que sur des questions portant sur la vaccination contre le rotavirus, nous obtenons un taux de réponses correctes de 9,77%, démontrant un manque de connaissances des médecins généralistes concernant ces vaccins. En s'affranchissant des freins principaux, à savoir l'absence de remboursement et de recommandations, 66,09% des médecins interrogés proposeraient la vaccination contre le rotavirus et 84,49% souhaiteraient avoir des informations complémentaires. Conclusion : Cette étude montre les principaux freins à la réalisation du vaccin contre le rotavirus en Ille-et-Vilaine, à savoir l'absence de recommandations et de remboursement. Si cette vaccination venait à être recommandée et remboursée, une information la concernant semble nécessaire pour une meilleure adhésion et une bonne pratique.

La gastro-entérite du nourrisson est fréquente et constitue un problème de santé publique. Le rotavirus est le principal vecteur de cette pathologie. La clé de voûte de sa prise en charge reste les solutés de réhydratation orale, mais leur prescription n'est pas encore systématique. La prévention par la vaccination anti-rotavirus est donc essentielle, mais les vaccins disponibles sont peu utilisés. L'étude a eu pour but de décrire et analyser les raisons de l'absence de proposition ou de refus du vaccin. Il s'agit d'une étude d'opinion auprès des médecins généralistes et pédiatres de la communauté urbaine de Bordeaux. Le taux de réponse est de 57 % : 419 répondeurs sur 732 (381 généralistes et 38 pédiatres). 71, 8 % déclarent avoir reçu une information sur le vaccin contre le rotavirus. Ils conçoivent à plus de 80 % la gastrro-entérite du nourrisson comme une pathologie sévère ou parfois très sévère. Pourtant le vaccin contre le rotavirus est rarement conseillé : 59,6 % ne le proposent jamais ou exceptionnellement, et seulement 2,6 % l'utilisent systématiquement. Les raisons les plus fréquemment invoquées sont le non remboursement (64,2 %), la surcharge du calendrier vaccinal (53,6 % et la non-recommandation du vaccin (35,1 %). De plus, ils pensent que les parents perçoivent la gastro-entérite du nourrisson comme une pathologie peu sévère à 44 %, mais qu'ils acceptent le vaccin à plus de 70 %. Ils invoquent comme principale cause du refus des parents, l'absence de remboursement (77,7 %), l'absence de recommandation (45,5 %) et la crainte des effets secondaires (44,1 %). La proposition du vaccin par le soignant est associée à sa perception de la gravité de la maladie. L'absence d'information est un frein à sa prescription. Enfin, ils modifieraient significativement leur pratique en cas de recommandation ou de remboursement du vaccin. Son avenir est donc conditionné à la décision des autorités de santé de le rembourser.

Countries that have made the most progress toward universal coverage have public expenditures in health equivalent to at least 6% of their gross domestic product (GDP), which is the percentage established in PAHO's universal health strategy as the benchmark for the countries. However, while higher expenditure is a prerequisite, it is not enough to combat inequities and advance toward universal health. In addition to greater resources, the quality of the expenditure must be improved, reducing health system inefficiencies. Moreover, public expenditure in health should be sustainably increased in a fiscally responsible manner. The concept of fiscal space for health refers to the ability of governments to provide additional budgetary resources for the health system without affecting the financial position of the public sector or supplanting other socially necessary expenditures. Any analysis of fiscal space, therefore, will attempt to identify the prospects for increasing health expenditure in the short and medium term to address a series of clearly established health needs. These efforts are under way at a critical time in the Region of the Americas, particularly in the countries of Latin America and the Caribbean, which are engaged in a singular health system reform process. For the first time in history, these countries have formalized their intention of increasing public expenditure in health, putting themselves firmly on the path to real and effective access to health care through the universal health strategy. Without achieving basic well-being at this level, it will be impossible to improve social cohesion and social development in the countries of the Region.
